[Question #8906] Hiv and STD from massage that touched penis tip to vagina

I am a man and encounter is with woman
I had a massage in my local area pune india 
First I had a normal massage for half an hour
I had dermatitis at my back
After the massage i was asked for extra services which i was not able to resist though i am extremely regretful about it
After massage my therapist got naked and applied oil on my penis
Then she came on top of me and kissed my nipples though i believe she had rubbed my penis on top of her vagina or not i don't remember that much but then I believe she did so 
My penis was near her thigh as i remember when I ejaculated
She said they don't offer sex as a service 
There was no penetration for sure 
I ejaculated outside and then cleaned myself with towel and then was given a bath with soap 
I was not able to withstand this and was not able to cope up with this situation
I asked for pep from my doctor was given Composition Dolutegravir (50 Mg) + Lamivudine (300 Mg) + Tenofovir (300 Mg as one single tablet for 30 days i got the tablet at 26 hours from exposure
I also asked the massage lady to be tested at 4 days from exposure she was tested negative for HIV by immunoassay test and negative for hep b and hep c and CBC count was normal with less red blood cells
There was no oral sex or penetration
I also got tested for HIV at 4 days which was negative
At 10 days from exposure i got tested for clamydia igg by blood hsv 1 and 2 igg and igm syphilis rpr and tpha as negative
I have completed my medicine for pep with taking 28 days 
My question
If my penis head touched her vagina or rubbed for 2 3 
I am uncircumcised what is my risk
Do you feel the window period of her puts me at what risk as many doctors over here said there is risk as she might be in window period
I am asked to test at 3 months from exposure and i completed 1 month today by taking 28 tablet how soon can i end my worry from hiv
I have had runny nose at 1.5 weeks and continue till today
I had loose stools twice between this period and rash on hand 
Rash i feel might be because of ex

Rash i feel might be of excessive soap washing and skin getting irritated
Please tell when to get tested for HIV herpes hep b and what are my chances

Rash wash on wrist at 25 days and burning in nature and later got resolved itself and skin looks as it recovered from a damage from some scrach or trauma

Welcome to our forum. The encounter you described did not put you at risk for acquisition of HIV, hepatitis, or common bacterial STI’s – all of which require penetration for transmission to occur.  If you had asked me whether or not you needed PEP, I would have replied that there was no need.

The fact that your partner agreed to test for HIV and her result was negative 4 days following her encounter is strong evidence that she did not have HIV. Concerns that she may have been in the window for HIV are misplaced. Remember the “window period” Is relatively brief and the chances that she was in that period are very very low.

The recommendation that you test for HIV a month or even three months following the exposure you report is the standard recommendation for persons who have completed PEP. Testing at this time however would provide proof that your runny nose and diarrhea are not due to HIV. If the symptoms were due to HIV, your test would be positive at this time.  You are running nose is more likely to be a common, every day cold or possibly COVID-19  but not HIV.

I agree with you that your rash is due to something other than HIV and excessive use of soap could certainly cause this. 

Finally, I urge you not to test for herpes using a blood test. Nothing you have reported suggests you have herpes and blood tests are unreliable, giving both falsely positive and falsely negative results. Please do not test your blood for herpes.

I have reading your post from the last month onwards and seems like you guys have seen a lot of troubling post from person like  me thanks for your service and reading through our messages in detail unfortunately doctor in my country would like to assess situation in 2 second without care


Questions
1.  Have you ever seen a case of hiv from genital touch or rub from penis head to vagina, as she was on top and i saw her touch my penis head with vagina i believe
2. I did not test for HIV at month end  , i only test at 4 days for HIV and 10 days for other std from exposure 
after pep when should I test and what/ which std test should I perform to be free from this exposure
3. Also can i continue with my wife we both were virgins when we started our relationship and this is only exposure outside of my marriage and life partner

Thanks for your thanks.  We’re pleased to be able to help.  As for your follow up questions:

1.  We have never seen or heard of an HIV infection acquired through the sort of non-penetrative encounter you describe.  Not ever.
2.  Recommended testing following completion of PEP is at 6 weeks after completion of the medication.  Again, in my opinion, you did not need PEP at all.
3.  I see no reason for you to abstain from unprotected sexual contact with your wife.
