[Question #8964] Handjob with Possible Secondary Syphilis small red bump on the CSW palm

Dear Drs
Hope you are well. Here is my situation.

I went to a premium club last night here in Dubai, the CSW was a Turkish, she gave me a Handjob twice with a lotion, i had shaved my penis and pubic area 4 hours before this happened. After the handjob i noticed a very tiny small red bump on her righthand palm that was painless to her and that's the hand with which she gave me the 2 handjobs. Now i am worrying after reading online about secondary syphilis transmission to me through that small bump on her palm, with my mind assuming she had syphilis.

When i asked her about it she said she has had that for almost 2 years and don't know what it is but she said she's never had an std before and if she had she wouldn't be coming to the club, she is a resident of Dubai and got tested 4 months back.

Do i need to worry about Syphilis/HIV etc? Her body otherwise was very clean, no lesions observed or seen across the body. She has a 5 year old daughter.

Could this red small painless bump on the palm be something else and not secondary syphilis?

Welcome to our Forum and thanks for your question.  I'll be glad to comment.  The single small bump on your massuese's hand you describe really does not sound like a secondary syphilis and further, even in the unlikely situation that it was, as long as the overlying skin was intact, infections would be most unlikely.

Most experts, including me will tell you that receipt of masturbation is a non risk event in terms of risk for STIs, including HIV.  That you'd shaved shortly before your encounter does not change my assessment.  Personally, I'd have np concerns about this encounter and not feel the need for testing.

Dear Dr EWH

My anxiety has flared up and I am panicking with the following thoughts:

1. You said it does not sound like secondary syphilis, can you explain why? (just to add her rest of the hands and body were fine)

2. How can I be sure that her skin was intact whilst masturbating me? Although I did not see any visible tear, or secretion from it or she seeming to be in pain?

3. I always thought hand jobs are safe sex events for STIs but your mention about the condition that the skin be intact has scared me further. I have read numerous posts on a handjob being no risk despite, cuts, secretions etc.

4. Should I be testing? And what should I be testing for?

Personally I don't want to go on a test cycle and elevate my anxious state.

Sorry for the panic and anxiety over this.

Sigh. I’m not going to argue with you. If you’re concerned and want to get tested do so. They reply to your specific questions:

1.  Secondary syphilis legends occurring on the pump are typically flat and relatively large. Further for a secondary syphilis lesion to of been present for two months would be most unusual.  

2.  You’ve just answered your own question. You do not see any evidence that her skin was broken.

3.  You are over reading this. Receipt of masturbation is safe sex. The presence of skin lesions on the hands simply do not result in transmission. Theoretically, if the lesion you have described, the description of which is atypical for secondary syphilis, were an open sore then perhaps, theoretically, transmission would have occurred. The likelihood that this happened is infinitesimal

4.  Weather not you test is up to you. In your situation, I would not test. If you choose the test, please plan to wait at least six weeks until you test so the results will be conclusive.

Finally, you are not doing yourself any favors by looking for information on the Internet. Much of what he said there is taken out of context, out of date, or simply incorrect. EWH

Sorry Dr EWH I did not mean to irritate you, I just want to summarize my understanding on this for the future and move on

1. So I agree that the lesion I described does not sound like secondary syphilis, however in the unlikely situation that it was does transmission of syphilis even occur this way or does it have to be oral, vaginal or anal sex (basically penetrative inside the body) since i understand the micro organisms cannot survive outside the host environment and infect?

2. How long do these secondary syphilis lesions remain seen on the palm of an infected person? are they single and isolated or at least a couple more present? just for my understanding to look for it next time I see someone.

2. The fact that you mentioned that you see no reason for testing is comforting. Thank you. Lastly just wanted to understand that if I don't see any symptoms in the next 3 weeks can I be rest assured that I did not catch anything from the 2 Handjobs?

1.I have never seen or heard of a person who acquired syphilis from exposure to a palmar lesion of the sort you describe in my >40 years of STI research and care.
2.  Like everything, the duration of lesions vary however most typically  secondary skin lesions remain for 2-3 weeks before resolving spontaneously
3.  If you had acquired syphilis from the exposure you describe, you would be expected to develop a lesion of primary syphilis ( an open genital ulceration) at the site of contact (your penis)
