[Question #9071] Finger licking after vaginal fingering

Hello there, i have a question that I am quite concerned about so i thought I'd ask the experts for advice. Ok today i visited an escort who i have seen numerous times before i done abit of fingering on her vagina & twice briefly took my finger out of her & placed it on my tongue.. She said she was clean i didn't see anything on my finger or even taste anything but after that we decided to do protected vaginal penatration. She got ontop & moved up & down for a couple of minutes then got of.. When she got of i noticed blood on the tip of the condom which looked briefly like had got in between the condom & the head of my penis which i couldn't understand but after touching the top of the condom i could see the blood was on the outside of the condom. I was abit shocked & said ' are u on your period? ' she said no that's happened because you are big & its difficult for me. So basically i was thinking if she was bleeding inside & i fingered her earlier & then briefly put the same finger on my tounge twice am i at risk for anything bad like hiv or the likes? As i say when i initially fingered her i seen no blood on my finger prior to placing it on my tongue. I'm going over things i my head here & its stressing me. The finger i fingered her with had no cuts but a very very slight layer of skin was peeling. Also im thinking about the condom it was on me & looked intact but as i say initially it looked like her blood had gotten under the condom but i think its just the way the condom was kinda ruffled up as i moved it the blood was on the outside.. Is there a possibility there could have been a tiny hole in the condom where her blood got in? I was always led to believe when a condom would break it would be obvious to see? Anyway thank you in advance & i appreciate you're help & advice.

Welcome to the Forum and thanks for your question. I'll be glad to comment. Before I get to specifics, let me remind you that most people, including most commercial sex workers do not have HIV or other STIs. In addition, even most single unprotected sexual encounters do not lead to infection. With that background, let me address your specific question.

First an importantly, HIV and other STIs are not transmitted from person to person when genital secretions or blood are transferred on hands or fingers. In addition, there are no data to suggest that ingestion of STIs results in transmission of infection- when people acquire STIs or HIV as a result of oral sex, they do so from DIRECT. not indirect ingestion. This that you touched your tongue following masturbation of your partner is not a concern and is a no risk activity.
Second, when condoms fail, they break wide open, leaving no doubt that they failed. Thus it is most unlikely that your condom failed. Your sense than any blood secretions present were on the outside of your condom is the most likely explanation for your observation. In addition, if your condom had failed it is most unlikely that your partners genital secretions would enter the condom. Rather it is more likely that material from inside the condom, such as ejaculate, would be transmitted to the outside of the condom.
Finally, I should qualify my comments above by pointing out the spotting during intercourse is a non-specific finding but that one of the associations described with bleeding such as you describe is the presence of genital chlamydial infection. Even if this is the case, with your condom use, infection would be most unlikely. That said, I understand that this fact may tip you in favor of having a urine test for chlamydia and gonorrhea.
Bottom line, the situation you describe is not one that I would be concerned about. Hope this perspective helps. EWH

Thank you very much Dr for your response i really appreciate that as i was just worried ( its a horrible feeling being worried & paranoid) yes i was surprised & shocked to see that 'spotting' ( blood on the condom) when i moved the condom it looked as if the blood was moving around on the inside of the condom which frightened me.. The girl put the condom on with her mouth & done oral on me for a few minutes prior to getting ontop of me i thought maybe her teeth may have damaged the condom. As far as the penatration goes she was only on top for maybe 2 mins or less & didn't even really move much. When she got of i grabbed a wipe & cleaned it & cleaned my hands i can't remember if i actually inspected my penis or not to see did any blood get on the penis head but as i say the condom was not bust wide open or anything. Based on this exposure would u recommend i get tested or not? Thanks again doctor for your time & response

Your major risk from this exposure wan not the condom protected exposure but your receipt of unprotected (if it was) oral sex. If the condom was not in place for oral sex there is a slight risk of oral gonorrhea and a miniscule risk for chlamydial infection. I am less concerned about the vaginal exposure. Testing is always a personal choice but there are several moving parts here including unprotected oral sex (if it was), the bleeding with genital penetration, and a somewhat riskier than average partner. Given that you may want to pursue a urine test for chlamydia and gonorrhea at some point more than 3-4 days after exposure (testing earlier carries a small risk of missing an incubation infection). Hi dr, no the oral she did on me WAS protected .. She put the condom on me with her mouth & did maybe 2 or 3 mins of protected oral on me then got on top for 2 mins or less then dismounted me that's when i saw the blood on condom & was concerned. Obviously I'd say HIV was my biggest worry but from what you say i shouldn't need be as worried as I've been ( all last night) about this encounter? One more question before i go.. With this particular girl i did NOT perform cunnilingus on her but maybe 3 or so months back i did like 30 seconds of cunnilingus on a girl ( a different escort) i only did it around the clitoris area, i remember dr handsfield kindly answered a question i had about cunnilingus about 17 months ago & he said its a very low risk activity for HIV & no known cases of hiv had ever resulted from such sexual activity.. as u know the medical world is constantly changing & new things are being learned & stuff i just wanted to know is that still the case that cunnilingus is extremely low risk & no known cases of hiv have been reported that way?. As you know, we provide up to three responses to each quiet question. As this is my third response this will complete this thread and the thread will be closed later today.
Thank you for the clarification regarding your exposure. The fact that the condom was in place for a receipt of oral sex makes this a Safe encounter and reduces my concern regarding your need for testing.
Regarding cunnilingus, there has been no change in evidence since the reply by Dr. Handsfield that you mention. There are a very small number of cases which is been reported as a result of performing cunnilingus on an HIV infected female. The rates of infection are so very low that they can only be estimated (as opposed to measured in carefully conducted scientific studies) and are estimated to occur less than once in every 10,000 exposures, if the partner happens to be infected (which is also unlikely). I would have a little concern regarding your risk for acquisition of HIV from performing oral sex on a female partner.
