[Question #9080] Possible Herpes?

Hello. I have been struggling with Chronic persistent urethritis ever since contracting clamydia over 2 years ago! Had a positive test as an NHS GUM clinic and I was treated for the infection but urethritis never went away. Additionally, I developed an intermittent but persistent tingling/crawling sensation in my scrotum and bladder pain (specifically in the morning before I pee - when my bladder is full). 

Over the last 2 years I've seen two urologists, had flex and rigid cystoscopies and taken multiple antibiotics to treat prostatitis (if it might be that). Nothing is working. Still have symptoms and they are very painful. 

Was never tested for herpes - could this be herpes? I've had no lesions.. just constant urethritis and the tingling. Let me know.

Forgot to mention... 

The urethritis doesn't really hurt when I pee anymore.. its actually more like a relief when I pee. Then the pain comes again after. Burning pain and sometimes the odd sting. 

I also get a very itchy scrotum.. it's actually kind of all over my genitals. particularly at night. I used to get it quite badly on anus too... 

The urologists have found nothing. 

I also tried acyclovir just in case it's herpes but it doesn't seem to do anything for me and just gives me side effects. Additionally, I've tried flucanazole and anti fungal creams too... To no benefit. 

I don't feel like this is anxiety related..  as these symptoms are incredibly real and painful. It's weird. 

Thoughts?

I very, very much doubt that this has anything to do with herpes.  With herpes in the urethra, you would have remarkable pain while urinating, as acidic urine passes over an open sore. The other difference is that herpes lesions in the urethra are not persistent - they would be painful intermittently, not chronically.  I'm sure sorry you are struggling with this but I don't think herpes is the cause.  The fact that acyclovir doesn't make it better also suggest that herpes doesn't have to do with this.

What would you think this is then? It all started after contracting clamydia... And now I just can't get rid of it... 

Prostatitis? 
A nerve issue?

Thanks for the response. 

But, surely HSV can exist even without lesions? Can herpetic urethritis be the cause just without lesions?? Or does herpetic urethritis simply not persist as it has for me for so long? Mine is unrelenting, everyday for last 2 years... 

Should I just get tested and if its positive assume that HSV is the cause?

well, I'm not sure.  Chlamydia in the urethra can disrupt some cells and cause pain, but it seems as though you are a way longer way out from that, than I would expect for you to still have pain.
Have you been tested for mycoplasma genitalium?  That might be one more possibility to check out.  Other than that, I'm afraid I don't have any thoughts about this.
