Good afternoon. Awhile back, Dr. Hook gave me some very helpful insight into my condom use and protection against skin to skin exposure that otherwise could create an STI risk. I had an extraordinary experience a few days ago and wonder whether if it might impact your analysis which, at the time, indicated pretty much no risk. I was in a bar in the Caribbean a few days ago that attracts locals, expats, and visitors and became engaged in conversation with a mid-30s woman from Haiti who had left the country due to the dangers there (her husband had been killed). She said she had worked as a nurse in Haiti, and her knowledge seemed to confirm that. Eventually, she revealed she was engaging in sex for money to support her 12 year old daughter and asked me if I would like to partake. I told her I knew she would not be doing this because she wanted to but because she had to. I then told her I did not want to exploit her situation and simply gave her the money she would have received if I had gone to bed with her. I ran across her later and asked how things were going (not so well). She eventually asked if I would like to go to bed with her without money being involved. After satisfying myself that she was being real, we went to bed and had cunnilingus and penetrative sex with a snug and properly removed condom (just a little gap between where penis and abdomen meet). Oh, she also had her mouth on my penis for about five seconds before I realized it was not the condom she was placing on me. It was clear from her interaction that she was genuine. However, I know that STI rates are above average in Haiti. Based upon this fact and what I have shared with you, do you believe there is any significant risk of an STI? Thanks for your insight.

Welcome back to the Forum. This was a very low risk exposure. For starters, it is unlikely your partner had HIV or other STIs, most people do not, even CSWs in relatively high risk countries such as Haiti. Equally importantly, your exposures themselves were low risk. Working through them exposure by exposure:
1. Condom protected sex remains safe sex with very, very low risk for infection. Not a concern.
2. Few STIs are acquired through cunnilingus, even if your partner happened to be infected. The estimated risk for acquisition of HIV through cunnilingus is less than 1 infection for every 10,000 exposures (i.e. 99.9% of exposures to infected partners would NOT result in infection).
3. As for your brief receipt of oral sex- again low risk for most STIs and there are still no cases in which a person has acquired HIV from receipt of oral sex from an infected partner.
Bottom line- I would not be worried about the exposures you describe. Testing would be very, very low yield and, in my opinion, not worth doing

Final question. I have genital herpes. Is there risk of acquiring oral herpes via partaking in cunnilingus (and is there significant risk of transmission of genital when no breakout)?

You are correct, I have little or no concern about skin to skin contact in the context of your encounter.
I presume that your genital herpes is due to HSV-2. If so, you will not acquire another infection and are protected by the immunity provided by your original infection. Your risk for acquiring HSV-1 is slightly but not entirely reduced by your immunity to HSV-2. Further, HSV-1 is not meaningfully transmitted through performance of oral sex on a partner with genital HSV-1. I would not worry. EWH
