[Question #9145] Protected Oral At Massage Parlor

Unfortunately I’m back yet again, On 15 August I visited and was serviced by two CSWs at a familiar Asian massage parlor. I did not engage in any penetrative vaginal or anal sex of any kind, in fact their pants remained on for the entire event although my hands and mouth were on their breasts and bodies while they both massaged me with oil.


I did however receive condom protected oral sex from both females separately. One performed condom protected oral on me which was applied with her mouth. This lasted approx. 5-7 minutes. At this point, the other female replaced the condom with a new one and performed oral sex on me until completion this lasted approx. another 5-7 minutes. Both condoms appeared to remain intact for both girls. (Each girl used a separate condom and performed oral on me  separately not at the same time)


Total encounter lasted no longer than 15-20 minutes. I have regularly visited these types of establishments in the past and am generally aware or the low risk associated with safe sex practices with CSWs, especially regarding condom protected oral sex.


Today is day 11 post incident while self examining my penis I haven’t found any abnormalities. I did however notice two small red raised bumps on my left thigh away from my penis . I chalked it up to pimples  or some other skin issue that can be found in the male inner thighs.  But of course my anxiety has me concerned.


I am looking for some reassurance and thoughts on this experience to quell some anxiety. I have began to apply a small amount of Mupirocin 2 % topical ointment. It’s worth noting that have been diagnosed with possible Hidradenitis suppurativa by my PCP.


I would imagine the incident above wouldn’t constitute the pimples I have now on my thigh which was not really exposed to any bodily fluids or vaginal skin contact, and I’m sure they may be a coincidence timing wise. I have already resumed unprotected sex with my wife as I figured this whole thing was fairly low to no risk. Just want some reassurance that this is a good move.

Welcome back to the forum. Thanks for being concerned about the character/word limit. But both versions of your question are visible.

Looking at thread a couple of weeks ago, you now describe a very similar exposure. As we discussed, there was little or no risk of syphilis, which was your main concern that time. We didn't directly address STI/HIV risk in general from such an exposure, but the implications were there. 

Even without condom protection, oral sex is safe sex -- not completely free of STI/HIV risk, but far lower chance of infection than unprotected vaginal or anal sex. In other words, you are exactly right with your own statement that "...this whole thing was fairly low to no risk." Going into more detail, I'm not at all concerned by "two small red raised bumps" on your thigh; undoubtedly these are unrelated to the sexual events. Mupirocin ointment was useless, with no effect at all in preventing any STI. (If anything, it might delay development of a syphilitic chancre -- but the risk of syphilis was too low for this to be significant, so no big deal. But I notice you reported the same thing last time as well. I would advise not doing it in the event of similar events in the future.

In summary, I see no need for STI testing, and there is no reason to stop having sex with your wife.

Appreciate your words of reassurance Dr.H.

This go around my concern is the acquisition of oral Herpes 1 onto my thigh from this incident. All of my common sense tells me this would be impractical because I only reached protected oral and aside from touching  and massaging my thighs were mostly out of play.

Chances of herpes manifesting itself after this incident and changes of it on thigh and no where else with only two spots.?

You are correct in understanding that lesions of a new HSV infection appear only at sites directly exposed. If you had acquired HSV1 from oral contact, it would have been on the penis, not nearby areas like the thigh. There isn't the slightest reason to suspect herpes as the cause of the thigh spots.---

Lastly, i would imagine the statistical chances of HSV 1 on my thigh from this event are probably less then 1%?

Appreciate your help and we can go ahead close this out afterwards sir!

I would put the chance at FAR under 1%. One in hundreds of thousands, maybe. That said, if the lesions become blister-like and/or other red bumps appear in a cluster at pretty much the same spot, it would be prudent to see a provider promptly for PCR testing for HSV1. However, I really don't think there is any realistic chance of it.
