[Question #9172] Oral STI

Hi doctors,
I was with a man on Friday 8/26 and the same man on Wednesday 8/31; on both occasions he rimmed me and gave me oral sex before topping me. ( we are both on prep)On Thursday morning 9/1, I had opaque white discharge. I went to the doctor who gave a urine pcr test for both gonorrhea and chlamydia; both were negative. He doesn’t have any symptoms but got tested for all Sti’s on Friday 9/2.
The doctor stated I had ngu and prescribed 1000 mg of azithromycin to take once. I took the pills and no longer have white discharge but see some clear sticky if I milk my penis.
1. I am assuming that the test would have been positive since I had discharge if I infact had chlamydia or gonorrhea. Can you confirm?
2. I also researched and wondered if I could have gotten a UTI since it came on within 12 hours and he had rimmed me before sucking me. If that is the case, would azithromicn knock it out.
3. Anything else I should be concerned about. I guess it could just be ngu due to regular mouth bacteria.

Welcome to our forum and thanks for your questions. Congratulations as well on being on PrEP and for being attuned to the possibility that PrEP will not prevent acquisition of other STI’s. You are asking the right questions and I’ll be glad to answer:
1. Current tests for Gonorrhea and chlamydia are very reliable. It is certainly most likely that you had non-chlamydial NGU due to the introduction of bacteria either from his mouth or his rectum into your urethra. This syndrome reflects your body’s reaction to the presence of foreign bacteria in your urethra and is not thought to be transmissible or have the same risk of complications as STIs like Gonorrhea or chlamydia. Azithromycin would be expected to resolve the problem as it sounds like it did.
2. From your description it is far more likely you had NGU than a UTI. The length of the male urethra makes UTIs uncommon.
3. I think you’ve done just fine. No need for further testing.

Doctor - just to clarify. I never inserted my penis in his rectum. He did give me a rim job and then gave me a blow job. Would ngu symptoms show up within 12 hours?

Understood. Receipt of oral sex can certainly lead to non-chlamydial NGU. The timing works as well. EWH ---
