[Question #9179] Syphillis Question

Hi Doctors,

i am a heterosexual male from the UK and live in Singapore. I am married and recentlu strayed with malaysian sex worker (who also lives in singapore). 

This was condom protected sex but i performed cunnilingus on her and received unprotected fellatio.

- Post this incident i had unprotected sex with my wife a week afterward. 
- i also took augmentin antibiotics (i believe these are amoxycillin) for something unrelated.
- i had no noticeable symptoms 

i have not yet tested and am concerned that if i given i have taken antibiotics that i could have infected my wife before clearing any infection i may have picked up.

Questions
- i am particularly concerned about syphillis. My understanding is that this is very rare in the heterosexual population and rare via oral sex (giving and receiving) - is syphillis a realistic concern?

( i am comfortable that the risk of other stds is too low to worry about)

Do you think i can move on without concern for my wife’s health? (on a risk basis)

Welcome to our four. Thanks for your question. I’ll be glad to comment. 

Your analysis of the situation is accurate, your risk of having acquired syphilis is quite low. As you point out, at the present time the global epidemic of syphilis occurs amongst men who have sex with other men rather than heterosexuals including heterosexual commercial sex workers. Further, most commercial sex workers do not have STI’s including Syphilis and as a generalization many commercial sex workers get checked regularly to permit early detection of infection if acquired.  In addition most syphilis is transmitted through genital as opposed to oral sex. When persons acquire syphilis they typically develop a painless ulceration at the siteof exposure (in this case this would be your mouth or your penis).  

The Augmentin you took, depending on when it was taken in relationship to your encounters, could well have prevented Syphilis if exposed or even cured it if the medication was taken for 10 days or more. 

Certainly testing is always a good idea and should you choose to do so I anticipate that you’ll find that your tests are negative.

Thanks Dr, That’s helpful.

Just to clarify, i meant that i had sex with my wife post this incident(unprotected so may have infected her) and then took augmentic (it was for 10 days). 

So, lets assume i’m personally in the clear. My question is whether the risk of me having caught syphillis is low enough is low enough not to worry about telling my wife and likely ending my marriage?

Thanks for the additional information. I agree that the risk of having acquired syphilis is quite low. I also suspect testing would be reassuring to you and would advise it but I do not see the need for you to notify your wife at this time. Hope this helps. EWH---

I don’t understand. If i have taken antiobiotics that would have
cleared the infection i don’t understand why it would be reassuring as the test would be negative.

Can you confirm you mean that if i choose not to test you are comfortable that the risk of ever having caught suphillis is low enough not to inform my wife?

If you’d acquired syphilis the syphilis blood test would still be positive.  Positive blood tests take months to resolve even after treatment (there are several types of blood tests.  Some resolve over months, some stay positive forever but none go away in just a few weeks).  A negative test would provide proof that you never had syphilis and resolve any concerns you might have about having had it. 

If you choose not to test that’s fine.  As I said earlier, your risk is minute.  EWH

ah got it! Thanks Dr - i had no idea. i thought if the infection had been cleared then the test would show as negative.Thats very helpful - thank you!
