[Question #9194] Risks and possible symptoms

Hello Doctors,
Thank you for your time and help. I am in need of some professional insight, I went to a clinic but they were in a hurry to just test me and get me out the door without answering any questions.. I recently had an extremely regrettable encounter with a csw that involved unprotected fellatio and cunnilingus for about 5 minutes, and protected intercourse. I am full of anxiety and regret as I am in a relationship, never ever again will I repeat this mistake. I did browse the forum briefly and understand that overall my risks are fairly low, but some things concerned the following days concerned me.
1. The next day, I noticed a persistent irritation and tingling on the tip of the urethra, no discharge, no foul odor, and what I wouldn't call dysuria but urinating had an unpleasantly warm feeling that I usually do not notice? On the second day I started to develop a sore throat that has improved since, also I was dealing with post-nasal drip from a cold over the week, which I understand can be the cause. I noticed the back of my throat however had 4 separate white streaks/discolorations going downward. Could these be anything to worry about at this time? Symptoms do appear to be improving at this time, about 5 days since exposure.
2. In an attempt for peace of mind, I went and got tested for G/C/Syphilis/HIV at 3 days post-exposure, awaiting results currently. I understand it's too early for both Syphilis and HIV, but would it also be too early to test for gonorrhea and chlamydia? I did a throat swab and urine sample for both.
3. Do you recommend me retesting at the 2-3weeks mark? Should I hold off on any intimacy with my significant other for now?

Thank you all again for your time and help, I truly appreciate this service that you guys provide.

Welcome to the forum. Thank you for your confidence in our services -- and for reading other threads with questions similar to your own.

Oral sex is low risk for all STIs and zero risk for some. There has never been a proved case of HIV transmission oral to penis, or by cunnilingus (oral-vaginal contact); and of course condom-protected vaginal sex is highly effective in preventing STIs, although not perfect. Also remember that most female sex workers have no active, transmissible infection at any point in time. So as you have already learned, all things considered, this was a very low risk event.  To your specific questions:

1. It is not possible to develop symptoms of any STI (or any other infection) under 24 hr after exposure; maybe by 36 hr for some infections (e.g. gonorrhea), but not before a week or so for chlamydia, and 3-5 days for herpes. And no STI causes nasal discharge, congestion, etc, and your sore throat also started too soon as well to be caused by any STI, including HIV. The improvement within 5 days also is inconsistent with any STI. It sounds like you caught a cold, nothing more -- maybe from the CSW, or anyone else in your environment. Or maybe COVID:  you should be tested for it.

2. Presumably your gonorrhea/chlamydia tests were on urine or a urethral swab, right? If so, 3 days is fine. You can expect a negative result (remember, you used a condom!), which will be valid and conclusive. Correct that HIV and syphilis testing were far too soon, except the negative results will provide a confirmed baseline, against which to compare conclusive testing in the future (assuming you do that).

3. You could repeat the HIV and syphilis blood tests at 2-3 weeks, but negative won't be anywhere near conclusive.  There is no need to repeat the gonorrhea/chlamydia tests. I don't see a need to stop having sex at home. If somehow I were in your situation, I would continue unprotected sex with my wife without any concern at all about possibly infecting her -- except with my cold (or COVID), although that risk already may be a lost cause, assuming you live with your partner.

I hope these comments are helpful. Let me know if anything isn't clear.
