[Question #9207] HSV 1

I tested positive by swabbing a lesion for genital HSV 1 about 2.5 years ago. I haven’t had another outbreak since until about 3 months ago. I did not take any antiviral meds at first because although I had lesions there were no symptoms and I was hoping it would go away on its own. After 2-3 weeks it was not going away so I took acyclovir (400mg 3x/day for 5 days). The lesions went away for a few days but came back. I took 2 more rounds of the meds. Same thing happened. My ob/gyn suggested I take 800mg acyclovir 2x/day for 3 months. I am on day 13 of this higher dose and I am still getting new lesions. Is this normal? As you can imagine I am quite frustrated. I will also add my husband gets cold sores. We have not had intercourse on the small chance he could get HSV1 on his genitals. We have only fooled around using our hands. A few times he used some of his spit. Is it possible the lesions are coming back because of his spit? Thank you!

This does not sound like HSV 1 genitally to me.  This is just too highly unusual that you would go all this time and then have persistent outbreaks.  And then the fact that you are continuing to get new lesions while on suppressive suggests that this could be something else.  Have you had a professional evaluate these lesions?

The new lesions seem to be smaller and go away faster. I have an appointment on tuesday with my ob/gyn. What else do you think it could be?

I'm unclear - perhaps a strep or staph - I have seen both genitally and they can appear like herpes lesions.  Also, a bad yeast infection can cause breaks in the skin that could be confused with herpes.  When you say they go away faster, how fast?

ok. I've had a yeast infection before, I don't think that's what this is. I know the burning, itchy feeling that comes with yeast infections and I have no symptoms at all. The latest small lesion that appeared in a new spot lasted about 5 days. There is one that always appears in the same spot and that one can take 2-3 weeks to go away. I was afraid the reason the outbreaks are so persistent is because I did not take an anti viral medication right away. I know it is best to take the medicine at the first sign right? Is it very unusual to continue to get new lesions while taking the anti viral? Thank you

It is not unusual to continue to get lesions after starting the medicine if you don't start it right away with the outbreak, no.  
If at all possible, please have any concerning lesions swab tested right away to know if they are HSV 1 or not.
Yes, always best to start meds at the very first sign of an outbreak to get the best results.

Ok thank you. I am waiting on results for a blood test to check for antibodies for hsv 1 and 2. I appreciate your help!
