[Question #9289] Thai massage with happy ending

Hi Dr,
I am a virgin. On 24th July I visit a Thai massage with handjob. Right after I had major regret and guilt, anxiety started. I felt itching, burning and prickling sensation in the groin area (scrotum, shaft, thighs, perineum, anus) lasting a week. Over next few days I developed rash on my body (chest, thighs, arms) and also single angioma type spots on chest and both biceps. On 11th September the burning, itching and pricking came back and still persists till today. I started noticing a lot of new bumps on my body. I got a filiform wart like growth on my shoulder, pinsize bump on my right thigh, penis glan seems to have more bumps, flat wart like bumps on forehead. I'm worried about HSV And HPV. 2 months after encounter I developed about 7/8 bumps in my groin creases, they were spread, single and smooth/shiny and I could flip it like skin tags. Two bumps in right crease were slightly raised brown like a mole but next day they were raised and grey like a skin tag. Also got a single skin colour bump on my lower left pubic area which resembled a wart, it was very small and could only see when zooming in with mobile camera. I saw 4 doctors and they all said it's not warts/HPV but didn't tell me what it could be. I have picked/scratched the growths and they went away. This all happened after the massage and handjob and I am almost certain that I have been infected with something. You mention massage/handjob is safe but I have physical symptoms from this encounter.

Welcome to our Forum. Thanks for your questions and your detailed description of your symptoms and lesions. You have been seen by 4 trained clinicians who have had the opportunity to examine you and who are unanimous that they see no evidence of HPV or other STIs. While I have not had the opportunity to examine you, from your description my sense is similar. The WHO, CDC, and all reputable public health organizations state that massage and receipt of masturbation, even from infected partners, is safe sex with no risk for STI. I concur.
I do note that you have experienced major regret, guilt and anxiety over your encounter. In my experience several things can happen in such situations. One is that persons tend to examine themselves more closely than they might have in the past, often noticing lesions and changes that they would normally overlook. In addition to this hypervigilance, in some instances anxiety and repeated self examination can lead to atopic dermatitis.
Bottom line, nothing you have described suggests an STI of any sort, including HPV. If one of the clinicians you have seen has not been a dermatologist, you might seek a dermatologist who might have a better and more specific diagnosis. Irrespective however, I urge you not to worry about an STI of any sort coming from the encounter you describe.

Thank you for your quick response
I have some questions:
1. When she was massaging my back I felt a rough bump, if this was a wart or herpes whitlow. Could it have infected me?
2. I also had brief itching and tingling on the back middle section of my ring finger about 3 weeks ago and then last week I noticed a tiny cut like I've grazed it and a bit of skin came off. This then scabbed up, could this be the start of a common wart or herpes whitlow? No swelling or fever.
3. If it's not HPV, how could I suddenly get these growths on my body. I am very concerned about the ones in my groin crease and the small one on my lower pubic area. These only showed up after the massage/handjob and exactly 2 months after. This makes me think I was exposed to the virus and the growths showed up within the average timeframe for Genital warts.
4. Can warts grow rapidly? I have been checking daily and these growths seemed to have developed almost overnight. I remember checking my right groin crease and I had no growths and then the next day I noticed the brown mole type lesion and the following day it turned into a skin tag type lesion. Could this be a reaction to something or could ingrown hairs cause such growths? As a lot of the bumps were close to hairs/follicles.

Again, this is a no risk event, hence answers will be brief:

3. These are questions for a dermatologist. What you describe does not suggest PV.
4. Warts would not appear overnight This is in no way suggestive of HPV. It is suggestive of hyper awareness related anxiety

Thank you for your answers, I do feel some relief now.
I forgot to ask one more thing. I was prescribed an anti fungal jock itch cream called Daktacort. Could a jock itch persist for more than 2 months and present without the typical red rash around the thighs. My groin looks normal but because of the burning, itching and tingling I'm thinking this could be caused by fungus? Could this be a possibility.

Itching can persist without an obvious rash, particularly in moist areas such as the groin. This is certainly not an STI. If the symptoms persist, you should ask your doctor to take a look
