[Question #9295] Worrying about my PCR test result after unprotected sex

Hello

I met with transgender sex worker for body-to-body massage, we gave each other oral, kissing and blowjob, and I preformed rimming on her.

Then she started rubbing my chest "she was setting on my lower stomach" then she grabbed my penis and and inserted the tip of my penis in her anus for no more than 10 seconds.

Three days later I started to worry, so I wanted to know her status by asking her indirectly, so I texted her and asked if she would have unprotected sex with me she said no problem but the price will be double, at that moment I realized that I am at real risk.

So After 14 days I tested for HIV ( I+II ) SCREEN - non-reactive which is I know too early for this test

next day at 15th  day I tested: Hiv by pcr - Not detected

Below was written in the results

**Lower limit of detection (95% detection rate): 13 copies/ml. The assay detects the subtypes of HIV-1 groups M, N and O.**

My doctor who is well known in my country and has more than 10 years experience treating HIV patients, told me this test is conclusive and no need further tasting, but I think he only said that because he thinks that I was paranoid about the sex worker I met that she inserted the tip of my penis in her anus. He said she didn’t, but the anxiety is playing with my brain, and I should relax.

My question to you

-how risky is my encounter?

- Is this test conclusive after 15 days ? ( in my country is conclusive after 9 days )

- How likely if I tested 4th gen combo after 45 days became positive?

- Does the Lower limit of detection (95% detection rate): 13 copies/ml. Has anything to do with the test accuracy?

- What is your advice?

Sorry my English is not my first language

Thank you,,

Welcome to our Forum and thanks for your questions.  I'll be glad to comment (and your message is quite clear- your English is fine).  Regarding your questions:

1. The encounter you describe was low risk.  I would have advised asking your partner directly and you might still do that but given the brief nature of the encounter and partial penetration, even if she had untreated HIV infection, your risk is low.  Less than 1/10th of 1%% of episodes in insertive rectal intercourse with untreated HIV infected person result in infection.  There is no known riskof acquiring HIV from kissing or receiving unprotect oral sex from an untreated infected partner

2.  - Is this test conclusive after 15 days ? ( in my country is conclusive after 9 days )
Your result is virtually conclusive.  Most manufacturers state that their tests are conclusive any time more than 11 days after exposure and I have never seen someone who had a negative test at 11 ofr more days go on to develop a positive test.  I would have confidence in your result.

3. How likely if I tested 4th gen combo after 45 days became positive?

Virtually zero risk that a 4th generation test at 45 or more days will become positive

-4.  Does the Lower limit of detection (95% detection rate): 13 copies/ml. Has anything to do with the test accuracy?

Typically when someone has acquired HIV their PCR is positive with thousands or even millions of DNA copies.  This limit of detection is effectively zero.  You can believe the result.

I hope this information is helpful.  Given your test results, I would not be worried in the least.   EWH

You can have complete confidence in your test result. There is no reason to abstain from unprotected sex with other partners. In addition, there is no need for further testing. EWH---

Alright I really don't want to be back here but for curiosity or anxiety, I'm here.

After seeing Dr. Handsfield replies about PCR test my anxiety went from 1 to 1o

Dear Dr. Handsfield, if you see this I'd like to hear your thoughts about my situation because I think doctors in general have different opinion about PCR.

Thank you

Dr. Handsfield will not be responding.  You are over reacting.  The exposure was low risk and you had a negative PCR at day 15.  Manufacturers state their tests are conclusive after 11 days.  Out of an abundance of caution some experts recommend repeat testing with a 4th generation test at weeks weeks.  That said, neither of us has ever seen or heard of a person with a negative HIV PCR at 11 or more days who went on to later develop a positive test.  You are not going to be the first and your test results are not going to change.

FYI, while our verbal styles differ, in more than 30 years of working together, Dr. Handsfield and I have never disagreed on the facts presented in our responses.  EWH
