Dear sir
 I had unprotected sex with a csw with high bleeding from penis frenulum. I started trioday within 48 hours and taken for 30 days. After 11 days of my exposure with csw, I got her tested where her viral load was 51 copies per ml but her 4th gen -ve. I got fever for only 4 days after finishing trioday and still having joint pain, muscle pain for last 40 days. It started on my last date of PEP. I had itchy rashes also for 2 weeks. However my 4th generation test as well as hiv1 DNA PROVIRAL shows negative after 35 days of finishing PEP(total 65 days after exposure). But my symptoms (muscle pain, joint pain, itches) not going. I tested my Cpk levels which were 300(lab limit less than 170) 5 days back(after 33 days of PEP). 

Pls help me as I have been staying away from my wife and I don’t know if my results are conclusive or not at the moment? 

Also I am healthy man with no other problem so why last 40 days my muscle pain and joint pain not going. My vitamin b12 level and vitamin d levels are almost okay as well.

Giving more info on my exposure: this girl intentionally pulled my skin down so there was one small cut on my penis frenulum. I am uncircumcised man. I was drunk and I only noticed it after finishing unprotected intercourse after seeing lot of blood, there may be blood from her vegina also because it was too much blood stains. And it was still bleeding from my penis frenulum after I finish intercourse. I came outside, not in vagina. So considering her viral load which I checked after 11 days (51 copies/ml, test detection limit was 20), I consider it high risk. She also took one medicine that evening which i didn’t ask which medicine. Her 4th generation was negative so may be she was on some ART medicine or PEP.

Pls help me.

The opening lines of your question appear garbled and I'm not sure I entirely understand the nature of the exposure, but I think I understand enough -- especially in view of your symptoms and your own test results -- to give you a very reassuring reply. You do not have HIV.

If indeed your partner's HIV testing is valid, from a virological perspective it was low risk. An RNA (viral load) result of 51 copies is extremely low number, probably too low for transmission. But her negative AgAb (4th generation) HIV blood test strongly suggests her RNA result is false positive. In theory, she could have had very early HIV infection, probably under 2 weeks duration -- but even in that case, her low RNA result probably is non-transmissible. When infected people are on effective ART, indeed it lowers the viral load, but effective treatment has no effect on HIV AgAb test results, which remain positive for life regardless of treatment. And so regardless of your exposure with a fresh bleeding injury of your penis, I would consider this a low risk exposure. In addition, PEP is highly effective when started within 72 hours of exposure. (You don't say what that interval was.)

But perhaps most important, your own test results are extremely reassuring. It appears your exposure has been managed by a legitimate HIV expert:  many providers would not have known to test for proviral DNA, for example. That result plus your negative AgAb result 35 days after completing PEP probably is conclusive. That said, some experts recommend final HIV testing a full 6 weeks after the last dose of PEP, and some even advise three months. However, considering all factors -- that your partner probably was not infected, her low viral load if she was, your completion of PEP drugs, and your post-PEP test results -- there is no realistic chance you have HIV. You can safely have unprotected sex with your wife.

Finally, your symptoms can be disregarded in relation to HIV. The symptoms of ARS are not caused by the virus, but by the immune response to it. Therefore, it is not possible to have symptomatic ARS in the face of negative antibody testing. That is, your negative AgAb test result proves HIV is not the cause of your recent or any current symptoms. In addition, I have never heard of musculoskeletal pain as the main or only symptom of ARS. Something else explains your muscle and joint pains.

My guess is your doctor has given you similar reassurance. But I do hope these comments are helpful and reassuring. Let me know if anything isn't clear.

Dear Sir, thanks for your reply and I do appreciate your help as I have been long time reader of this forum now.

anyways from your reply I understand you didn't get my 2nd msg where I mentioned in detail about my exposure please see again as below (pls don't count it as follow up msg hence)-

Now my questions are as below and beg you to reply point wise. I know most of the people are just having HIV phobia so you need to give them consoling answers but I want honest answers so that I can 

1) Her viral load was 51 copies after 10 days of my exposure, so we do not know what was her load at the time of my exposure with her, also I do not know whether she took any hiv medicine (PEP) before  having sex with me (as I discussed explained above) also I got her checked from top lab of india and lab people said that there is no possibility of false positive results from their side because its FDA approved test and machine gives the results, so no chances of wrong results. Here some doctor said that may be she have very acute infection. Please your comment on this?

2) Also as I am uncircumsized and there was bleeding from my penis frenulum during intercourse, I have read that penis frenulum have HIV receptors so I believe I am on high risk. My intercourse with her lasted around 15 minutes approx. I saw lot of blood on bed sheet after I finished my intercourse, so I am afraid if she was bleeding too during the intercourse, I could not verify it. Please comment on this.

3) I was prescribed Trioday (TLE) as PEP which I have taken for 30 days, also I didn't maintain proper time for it as everyday the time changed 1-2 hours plus minus. For first 15 days, I have taken it at morning and after that I started taking it at night. later on, I came to know that Trioday is old medicine with side effects and now no one is use it as DTG is much better than Efavirenz. But my doctor was not hiv specialist and I did not have any idea so I took it. 

3) On top of it, I have had fever and chills for 3 days after PEP last day and body rashes with itching for 3 weeks. Still sometime I feel someone pinching my upper arms while I sleep and it gives burning sensation. and  I have itchy red bumps on my body specially hands. Also my muscle pain and joint pain is not going last 40 days despite I have tried everything and now also taking medicines for that. I have also tried several physiotherapist sessions. Here many doctor says that long term muscle pains are symptoms of acute HIV. Pls comment on this. 

4) Now coming back to tests, I have tested negative for 4th generation(Ab/Ag) test latest yesterday means after 40 days of finishing PEP. And HIV 1 DNA proviral negative after 33 days of my last dose of PEP. (63 days from exposure date). Please comment on this.

5) Please advise me if HIV 1 DNA proviral qualitative test is better or HIV 1 RNA qualitative test is better for HIV early detection of HIV? And these should be qualitative or quantitative? AND WHICH TEST AND WHEN TAKEN WOULD BE FINALLY CONCLUSIVE AFTER PEP. I HAVE NO PROBLEM IN SPENDING MONEY AT THIS MOMENT.

6) I know mostly people coming here just have HIV phobia and you need to give them consoling reply, but I believe I am in true danger because of my bleeding, unprotected sex, girl positive, taking wrong PEP medicine and now symptoms. So I want honest reply so that I can stay away from my wife from unprotected sex and can save my family, I have kids too and I am just 32 years old.

Please reply point wise.

1) I still doubt your partner has HIV. Even if she does, your tests prove you didn't catch it.
2) This makes no difference in my assessment of your situation.
3,4) As I said above, it is not possible to have HIV symptoms in absence of HIV antibody and other evidence of HIV infection. You do not have HIV; something is the cause of these problems.
5) You have already had completely conclusive tests. I am aware of no other tests you could possibly have.
6) Whatever "danger" there was of HIV at the time of exposure no longer matters, because it has been proven you do not have HIV.

I am sorry but I expected detailed reply as I have listed down so many things and some questions were not answered specially testing time after pep. Pls try to give detail reply. I am sorry from my heart but I need this.

Also pls give me hiv testing conclusive guidelines after PEP for HIV DNA/RNA testing (I also asked which test better in DNA and RNA). Pls I want to keep safe my wife and children and I need to do family planning for my 2nd child. I hope you understand my situation at the moment.

I'm sorry, but the bottom line is that you do not have HIV. All other questions are therefore irrelevant, and I see no need to explain my reasoning in any more detail than I have.

There are no "conclusive" guidelines for testing after PEP, i.e. no official agencies have published or otherwise promoted conclusive guidelines. However, most experts do not use genetic (RNA/DNA) testing, only AgAb tests. Most agree that those results are nearly conclusive 4 weeks and truly conclusive 6 weeks after the last dose of drug, but some recommend testing a final time at 3 months.

That completes the two follow-up questions and replies included with each question and so ends this thread. If you remain concerned or uncertain, I would advise you to go back and re-read all my replies, carefully. If still concerned after that, ask the doctor or clinic where you received your PEP. Most likely they are equally expert as we are on this forum.

Even though you obviously doubt our expertise or knowledge, I hope the discussion has been somewhat helpful. Please understand you do not have HIV. That's all that matters, right? Good luck to you.
