[Question #9332] Oral Sex & STD Risk.

I am a late 30's 'straight' male (married) who had mutual, unprotected fellatio with a gay man in his mid-30's last week. He was vaccinated against COVID-19, Monkeypox, and Meningitis. He is also of negative HIV status, and on Prep.

We performed unprotected fellatio on one another for a brief period of time (couple of minutes). He did not ejaculate into my mouth, in me, etc...

Given my paranoia surrounding the situation, I took a 1000mg dose of Cefuroxime that I had on-hand, as well as a 1000mg dose of Azithromycin.  

Given the circumstances, I am quite anxious about my status, and am refraining from any sexual contact with my wife until I gain further understanding of my potential exposure here.  From what you've read, what am I at risk for from an STD standpoint?  Syphilis is of concern, simply because it isn't prevalent in the heterosexual population. Obviously, most oral chlamydia/gonorrhea would be cured given the antibiotics I took. Thank you for reviewing this.

Welcome to the Forum and thanks for your questions.  I'll be glad to comment.  Amongst penetrative sexual activities, oral sex tends to be lower risk than others but not no risk.  There are no proven cases of HIV acquired from oral sex and acquisition is chlamydia is quite rare.  Gonorrhea and non-chlamydial NGU are the most common infections transmitted through oral sex and syphilis is rare but occasionally acquired in this way.  The drugs you took should prevent gonorrhea, NGU, and syphilis.  While we do not endorse taking antibiotics preventatively as a rule, the drugs you took make this an essentially no risk event.

  Thanks for the response. From your estimation, given some of the data surrounding azithromycin-resistant cases of syphilis in studies....is that situation a risk from your experience, or not so much? I'm located in the USA, in the south of the country.

While there is a modest risk of, azithromycin resistance IF you were exposed (which is unlikely) the cefuroxime you took is also active against the syphilis bacteria.  Hence, you are in the clear in the unlikely circumstance that you were exposed.  EWH---
