[Question #9379] HIV/ STI Concern

Hello, thank you for all you do. Male 37 yr old exposure with worker almost 4 weeks ago unprotected oral protected vaginal but condom broke at some point and noticed a little blood from her. Tested negative full panel 4 days from exposure which I realize is too soon. She seems to be fine and showing no symptoms. Immediate symptoms starting at 2-3 days from exposure : loss of appetite (has gone away) urge to urinate more, extremely thin clear discharge after urinating which doesn’t happen often and always stays in urethra. I have not had any pain burning or itching while urinating at all, but at very few times something doesn’t feel right down there . No sores or lesions. No rash or fever. No night sweats or swollen lymph nodes that I’ve noticed. I’ve only masturbated twice since exposure and no pain ejaculating but the tip of my penis gets a little red which I can’t tell if that has always happened or not. Treated w doxycycline, a shot in the back, and metronidazole, doctors think it’s not herpes related but in my head I think it’s either HIV or hsv. I have been severely depressed and anxious about the whole incident and examining myself 24/7. 

Do you think this is HIV or HSV? Is there any chance this could be prostatis? How soon can I take a western blot to try to get peace of mind? Any insight is greatly appreciated.

Welcome to our Forum and thanks for your questions.  The symptoms you describe are not suggestive of any STI.  Urinary frequency is not an STI symptoms, a small anount of clear urethral moisture/discharge in normal and STI symptoms do not come and go.  Further, the therapy you had would have cured an STI if present.  Your symptoms are in no way suggestive of either HSV or HIV.  

You may be playing closure than usual attention to otherwise normal sensations.  I urge you not to worry.  I see no need for further testing.

Thank you for the quick response. Again I appreciate all that you guys do you provide relief for people who are desperate for answers. 

I woke up this morning and had noticed the same clear discharge at the tip of my penis. Might be too much info but I’m hoping it’s from arousal from a dream I had because I have urinated a few times since and have not noticed any discharge. I do feel like at times I feel a slight discomfort in my urethra but I still have no issues urinating in terms of pain or burning. I have been to a urologist who didn’t seem at all concerned with my symptoms, but given the exposure and the fact that things haven’t felt right down there since I am still extremely worried in my head that this is something. Is discharge in relation to herpes usually only present if sores or lesions are present ? Is there anything I can do or take to get rid of this problem and to start feeling normal again? I am definitely feeling better that you seem to think this is all nothing but is there anything else you think could be the reason for me feeling this way? I just feel like it’s weird given the situation to have these symptoms post exposure and not have any issues going on. Thanks again and I look forward to hearing from you!

As I mentioned in my original reply, a small amount of moisture and clear discharge is perfectly normal. Most people do not notice it unless they are specifically looking for it. The fact that your most often occurs in the morning on awakening is also not uncommon.  As I also mention to you the treatment that you received, while probably not necessary, would have cured all of the common causes of urethral discharge. I would not be worried in the least about herpes. Herpes of the urethra is typically quite uncomfortable most often accompanied by lesions.  

Finally, I would remind you that you have been seen by a train clinician who also feels that there is nothing to worry about.  Try your best not to focus so much on general sensations. EWH

Thank you for your response once again. I appreciate all the information you have provided. Do you have any information on what this could possibly be? Given the timing and the symptoms I am experiencing I still can’t help but think that this has to be something going on. I’ve been experiencing these on and off symptoms for almost a month now. I have never really had any burning or itching anywhere but discomfort in the area comes and goes from different spots and I feel like that attached with the discharge I am noticing has to mean something is going on. I have experienced minor discomfort in the urethra at times but never while urinating. My urologist didn’t rule out hsv but didn’t seem to be worried. If I took a blood test now for hiv and hsv while having these intermittent symptoms for almost a month would the results be accurate? I am trying to take your expertise and your opinion on the matter and move past it, but it is very hard for me to given the circumstances. I’m sorry to be a bother, but this has been eating me alive.

These will be my final comments. We provide up to three replies to each clients questions and as this is my third response, this will complete this thread which will be closed shortly.

Turn off a lot of your follow up is repetitive and has been already addressed. For reasons that are unclear to me, you appear to be quite anxious about the encounter you’ve described. As I have already indicated, when persons are looking hard for symptoms ( often due to guilt and/or anxiety), they tend to observe otherwise normal sensations which they went over otherwise overlook. This may be contributing to your issues.

Neither the symptoms of herpes nor the symptoms of HIV come and go as yours have.  Your testing for HIV is 99% conclusive following four weeks and entirely conclusive at any time more than six weeks following the exposure. I remain confident that you do not have HIV or herpes.

At this point, my primary suggestion would be for you to discuss your issues, your test results, and the information you have received from trained professionals as you tried to work past your unwarranted concerns.
