[Question #9454] Prostatitis vs. STD from Unprotected Oral

Hi.  Thank you for the service you provide.  I have spent a ton of time reading your answers

I am heterosexual, married, mostly monogamous.  I recently had an encounter with an existing occasional partner (female) and she performed rather vigorous oral sex on me.  I did not ejaculate.  I woke up concerned and decided to take two days worth of doxycycline - I know that was not enough but I was bit desperate.

Approximately 4 days after this encounter, I had unprotected sex with my wife.  The following day - so on the 5th day after the encounter, I started to have some pain in my testicles.  I do have chronic prostatitis that flares up occasionally.  I have had no discharge or real pain in urinating, but some pain that could be either testicular or prostate.

I went and got tested for chlamydia, gonorrhea, and HIV at my local std clinic.   When my nurse was checking my urine, she commented that" there was a lot of mucus" there.  I immediately googled mucus in urine and saw that both chlamydia and gonorrhea are likely causes. Results due back in 2-3 days.  I have an appt today to be tested for NGU, chlamydia and Gon.

I have read many posts that state the most likely stds from unprotected oral are gonorrhea & NGU.  I noted the specific study from Dr. Hansfield conducted on chlamydia transmission to the male receiver as being below 0-1%.  So, I am optimistic this is not chlamydia.  

My questions are as follows:
1) What do you think I could have contracted?  Do you agree with my assessment that it is likely gon or NGU.  I honestly think it is unlikely gonorrhea given that I have had no discharge or pain -  no " pissing razorblades" per Dr. Hansfield.
2) Could this just be prostatitis? And anxiety?
3) Is the timeframe consistent with an NGU infection or is it too soon to have symptoms?
4) Does NGU cause testicular pain? Could NGU cause mucus in my urine?
5) If this is NGU and not chlamydia, do I need to tell my wife and get her treament? Biggest concern, honestly.

Welcome to the Forum.  I'll be glad to comment.  This was a low risk encounter and, based on a very recent study the medications you took would eliminate any risk for chlamydia and syphilis and reduce your risk for acquiring gonorrhea by about 50%.  My sense is that the discomfort you experienced occurred in both testicles.  if so, this too is not suggestive of any STI but could well be a manifestation of chronic prostatitis.  Also, Google is not doing you any favors here- "mucus threads" are not a sign of any STI.  Finally, as an FYI, prostatitis is almost never due to STIs.

Thus, with respect to your specific questions:

These symptoms really do not suggest an STI of any sort for all of the reasons I mention above.

2) Could this just be prostatitis? And anxiety?
This certainly could be a manifestation of your chronic prostatitis or heightened awareness of otherwise normal sensations relative to your guilt/anxiety.

3) Is the timeframe consistent with an NGU infection or is it too soon to have symptoms?
See above- this really does not suggest NGU.  If this were NGU, I would expect that your urine would have been positive for abnormal amounts of white blood cells.  If they were present I presume you would have mentioned it.

4) Does NGU cause testicular pain? Could NGU cause mucus in my urine?
No to both questions.

5) If this is NGU and not chlamydia, do I need to tell my wife and get her treatment? Biggest concern, honestly
Again, I think this is not NGU.
