Hello Doctors, i wanted to ask about test post PEP, ive been here before. my exposure was with a female CSW/escort. i had a condom breakage and was probably at most inside for a few seconds. i am unsircumsized and maybe had a few small nicks or cuts from shaving about an hour and a half before the event. i was the insertive partner. i went on PEP at the 65 hrs post event mark. after being on PEP for about 2 weeks i had protected sex with my girlfriend, but the condom was slipping up a little but i still didnt see any leaks from the condoms. She started her own PEP the day after that event which was at about 24 hrs after. we both have gotten tested i got tested at 4 and 7 weeks post PEP with a duo test and at 9 weeks post PEP with an HIV RNA QUALITATIVE test from LABCORP all tests came back negative. my girlfriend got tested at 5 weeks post PEP with a duo test and came back negative however she got a bit sick at about 6 weeks post PEP she got a bit sick again mild body aches, tiredness, some congestion, no fever, no lymph nodes. she did get a small itchy rash only on the left side of her pelvic area and shes also had a lingering cough for about a week and a half. my questions are
1) Did my girlfriend go through ARS.
2) what is the threshold of the RNA Qualitative test from labcorp?
3) are my tests conclusive yet including the non reactive RNA test 9 weeks after the last dose of PEP?
4) what are the chances that i infected her?
5) would a negative rna and duo test at 3 months post pep be conclusive? do i have to wait that extra month?

I also do want to add that at the time i got my 9 weeks post pep test 3 months post exposure i tested Negative for everything else including HSV, HEP A, B, C.
gonorrhea, syphilis, chlamidya, all negative.

i am very sorry, one more thing, i myself have had stomach discomfort for about a week and a half including 2 days before my negative rna test. can that mean anything?

also what worries me is that i didnt get sick from her being sick, which makes me think it wasnt a common cold, flu or covid

Welcome back to the Forum. On this occasion I happened to pick up your new questions rather than Dr. Handsfield. I have reviewed your earlier interaction with Dr. Handsfield and agree with all that he said. Much of your current questions are repetitive although you have now had additional testing which PROVE that you were not infected from the low risk encounter you described. Since you certainly do not have HIV, your GF cannot have been infected by you either. Having provided you with the essential answers to your questions, I'll now go on and answer your specific questions:
1) Did my girlfriend go through ARS.
You do not have HIV. Thus you could not have infected your GF and her flu-like symptoms were not the ARS unless she has had other partners. It is far more likely that her symptoms were due to one of the far more common, every day viral illnesses such as influenza, COVID-19 or some other community acquired virus.
2) what is the threshold of the RNA Qualitative test from labcorp?
I do not know the threshold of the LabCorp test but most likely, and similar to other commercial assays, it would detect HIV in concentrations of more than 50 copies. If you were infected your viral load would be orders of magnitude greater than the threshold. You should believe your HIV RNA PCR result.
3) are my tests conclusive yet including the non reactive RNA test 9 weeks after the last dose of PEP?
Yes, your combination of test results are absolutely conclusive. You should believe them. There is no need for further testing
4) what are the chances that i infected her?
Zero
5) would a negative rna and duo test at 3 months post pep be conclusive? do i have to wait that extra month?
Your current results are conclusive. You should accept and believe them and move forward without concern. There is no need for further testing.

hello doctor, about 2 weeks ago on december 18 to be exact, i woke up feeling sick, with a tension headache, a runny nose, lost my voice a little and a bit of nausea. my girlfriend also had these symptoms before me about a week or 2 before, except she actually vomitted and felt better right after, but she would get nauseas often for about 5 days until she vomitted. i am concerned because since then shes had these spasms in her eye constantly and i wanted to know if HIV could be the cause of it, not as ARS, but maybe as a chronic thing keep in mind this would be right after she was sick. i hope that it might be related to stress and not HIV. another concern is that both of us have small amount of skin peeling around our toes no redness nor itchiness but when i was looking it up it said it could be psoriasis and when i looked up psoriasis i saw that it could be related to HIV, both in the early stage and in the late stage. one more thing is that personally have felt itchiness around my body with no visible rash anywhere, my back gets itchy, my shoulders, my buttocks, my neck, my legs,my feet but not where the peeling is around the toes as well as my arms and hands. i was calm and at ease with the tests ive done and all negative but i started getting these symptoms around 2 weeks after my negative Qualitative PCR test 9 weeks post PEP. i started the cycle again with getting worried. what are the chances that my qualitative 9 weeks post pep was a false negative and thats why i am getting these symptoms?

My girlfriend has also had this lingering cough where she will cough out of nowhere.

I’m sorry to hear that you are having so much difficulty moving forward.
As you’ve already been told, your tests absolutely prove that you were not infected with HIV. There is no reason for further testing. From the sounds of things and remembering that it is cold and flu season, you and your girlfriend likely had a common, every day, non- HIV, non-STI, respiratory tract infection. This is not HIV. There is no need for further testing.

For my peace of mind i went ahead and had another qualitative and ag/ab test 3 months and 10 days post PEP, 4 months and 10 post exposure. they were both Negative. Is that good?

Yes. Just like your earlier test, this test proves you did not get HIV from the encounter you described.
