[Question #9487] Chances of HIV and other std.

3 1/2 weeks ago had protected sex with prostitute at shady massage parlor in Hawaii. She provided the condom so not sure of quality. She gave me oral with a condom then had insertive sex with condom. Sex did not last long and I did not cum. Went straight home and took a shower and my partner wanted to have sex, so I did because I had used condom with prostitute. 5 days later gf went to ER and was diagnosed with uti and throat infection. She had a fever, high heart rate and sore throat. My gf has pots so she already takes heart rate medicine but it hasn’t been working. They gave her Cephalexin . Sore throat and white blood cells in urine seemed to have cleared up after a while , but she still had fever and high heart rate… I went to urgent care the next day to get checked out of fear it was because of me. This is 6 days after exposure. Found white blood cells in my urine but culture showed no growth and chlamydia, gonorrhea, and trich was negative. Took doxycycline until had no more white blood cells in urine. Me and gf have sex again. She’s goes back to doctor because her heart rate has still been high and they say she has white blood cells in urine again and a fever. Ive told her to take doxycycline and I’m retaking it as well. I have just gotten retested today 3.5 weeks after exposure for everything. I am worried because I just checked my mouth and on my cheeks behind my top teeth I have some 2 small circle that look like lesion but I was told they could be salivary glands. I have also had a discomfort in my throat and tense neck muscles along with some tense back and tense/weak leg muscles but this could be from me not eating due to stress and anxiety. Can I trust my first test results for chlamydia, gonnorhea and trich? Any worry of HIV?

All test have come back negative. So Nevermind that. I guess my new question is what could be causing this? I guess we will both keep taking doxycycline in hopes this fixes the issue. Thoughts?

Welcome to our Forum.  Thanks for your detailed description of your situation.  I'll be glad to provide some comments. I think you need not worry about gonorrhea, chlamydia or trichomonas as long as your urine specimen was taken correctly (i.e. the first part of a urine specimen, not a mid-stream, taken at least an hour after you last urinated).  Current urine tests for the organisms you were tested for are highly reliable and give conclusive results at any time more than 3-4 days after an exposure 

Your exposure was low risk.  Most commercial sex workers, even the "shady" one, do not have STIs (or HIV), even most unprotected exposures do not lead to infection, and a condom was used.  As long as the condom was used throughout your exposure and did not break, you can be confident that it worked.  When condoms fail, they break wide open leaving no doubt that they failed.  

The white blood cells in your urine suggest you may have gotten non-chlamydial NGU from receipt of unprotected oral sex. This problem is typically not a risk for partners and doxycycline should treat it effectively

The fact that you both now have or have had sore throats suggest that at least part of what is going on here is an "everyday" non-STI  viral respiratory tract infection of the sort most people get from time to time.  Since you did not perform oral sex on your CSW partner, your sore throat, (nor your GF's) would be due to the exposure you described.  Similarly, the lesion you describe on  your cheek do not suggest an STI- they suggest that you are looking closely and may have what is called aphous ulcers which are not STIs.  

Bottom line, it is unlikely that what is going on here is an STI.  It might be worthwhile, and reassuring to suggest that your GF be tested for STIs but I doubt that is what is going on here.

Well she has white blood cells in her urine, and has been running a fever along with high heart rate. The doctor said her urine looks contaminated. That’s why I figured we should take doxycycline to see if that fixes the problem. Do you have any other thoughts or suggestions?

The doxycycline is unlikely to cause problems but I suspect there is something else going on here.  She needs to work with her own doctor to determine what is going on and should ask him/her about their level of concern.  EWH---

I think I left something important out. They’re is bacteria in her urine along with the white blood cells. When cultured the bacteria show no growth though. I believe the same bacteria was in my urine as well. She has been taking the doxycycline for 7 days now and we checked her urine again and most of the bacteria is gone, but she still has a fever with no other symptoms . I’m hoping the fever breaks in the next day or 2. The only symptoms I have been having is some slight itch discomfort in my penis every now and then. Any thoughts on what this bacteria is?

No idea what the bacteria in the urine are but I am confident they do not represent an STI and are not related to the encounter you describe.
