[Question #9495] HIV test 4th gen Duo lab

Hi I am 31 , I had a drunken night on October 7 2022 , I wasn’t sure if anything happened, but remember waking up in my car in the morning, I also didn’t see anything unusual around me.. I inspect my car and my paint but didn’t see anything suspicious. Afterwards I couldn’t stop thinking of that night so I took a hiv 4th gen duo test at 20 day post exposure (none reactive), then after I came across some website online where it says it’s on 80-90 percent, then I took another 4th gen duo at 6 week came back (none reactive ) then did a little more research because I was having symptoms, running noise clogged ear and felt slight swollen lymph nodes under my jaw so I got me anxious and when to the ER emergency and got another antigen/antibody at 9.5 weeks blood from vein lab where it was (none reactive ) is my results conclusive?

Is 3 -6 month needed to be conclusive ,what’s with the 99.7 by 6 weeks . But not 99.9 percent ? I been restraining from have unprotected sex with my wife .for the past 2 and a half month.

Welcome to the Forum. Thanks for your question.  From your description I find myself wondering if you had any sort of exposure at all on Oct. 7.  Irrespective, you have proven that you did not acquire HIV at that time. I'm not sure where you are getting your information.  If it's the internet that would explain the variable recommendations as much of what is found there is incorrect- either out of date, taken out of context or just plain wrong.  Experts are in agreement that results of 4th generation, combination HIV antigen/antibody tests provide entirely conclusive results at 45 days and most experts use 6 weeks (42 days) as their cutoff for conclusive results.  Tests taken at 4 weeks detect about 99% of infections and neither one of us on this forum has ever seen or heard of anyone who had a negative 4th generation test at 4 weeks and went on to become positive unless they had taken anti-HIV drugs following exposure.  

I urge you to believe your results and move forward. There is no reason for additional testing.  You have proven you did not acquire HIV on Oct 7.  EWH

Thank you Doctor  , I did not take any sort of prep medications. So you’re saying my results are 100 percent prove I don’t get hiv . 
I would like to know what’s the percentage of hiv antigen/antibodiesin by blood 6+ week difference from 3 month , ,
1 month before I got last blood draw for hiv antigen/antibodies I also when to a strip club where the dancer was grinding on me she did have a G-string on and I had my pants on  , but I got worried again because I seen some fluid on my  pants near me penis  where she was grinding can the fluid reach to my urethra and I  get hiv , sorry for being annoying I have a hiv phobia ever since October 7 . That’s all I have please if you can answer these questions I can move on. Both you Doctors are a amazing, God bless ☺️

Your reply is a bit repetitive.  As I told you, at 6 weeks tests for HIV are completely reliable.  i.e. there is no difference in the numbers of cases of recently acquired HIV infections diagnosed at 6 weeks as compared to 3 months- none!!!  Persons suggesting that one needs to take 3 months for definitive results are incorrect and out of date.  

"Having an exotic dancer "grind" on you is a NO RISK event.  No penetration, no risk.  It is for that reason that mutual masturbation, even when partners get each others genital secretions on one another, is a no risk event with no need for testing.

Doctor I have a question for the 4th gen antibody test planned Parenthood say’s 3 months , Are  they referring to rapid test only ? This is the website https://www.plannedparenthood.org/learn/stds-hiv-safer-sex/hiv-aids/should-i-get-tested-hiv I even called there office and the nurse was saying 3 months to be conclusive now I am worried again . What’s the difference from 4 gen antibody vs antigen/antibody test lab work .  I know everywhere it says 4th gen rapid test are 18 to 90 days after exposure. But  antigen/antibody test lab  18 to 45 days after exposure.I also got a test done at 9.5 week lab work ) none reactive

This will be my final response.  This reply indicates continuing unwarranted anxiety over your low risk exposure.  After this response, this thread will be closed.  Should you return to the site with further repetitive, anxiety-driven questions, your question may be closed without a response or return of your posting fee.  

4th generation tests are NOT antibody only tests.  $th generation tests test for both antibodies to HIV and antigens (the protein coat of the virus) from the virus itself.  Antigen levels rise before detectable antibodies may be present.  For that reason 4th generation tests provide conclusive results sooner than antibody only tests.  4th generation, combination HIV antigen/antibody tests provide conclusive results any time more than 6 weeks/45 days after an exposure.  Tests which test for antibodies only are not conclusive until 8 weeks (56 days) following exposure.  

Your antigen/antibody test at 9.5 was conclusive and did not need to be repeated.  You should believe the result and move forward- not waste you time and resources with further unnecessary testing.  EWH
