[Question #9513] New Questions

Hello doctors, sorry to come back but I have a few other/new questions and discoveries that I want your opinion on
1) Its been now 9 months since my exposure and I repeated my Ag/Ab HIV tests and another set of RPR and FTA-ABS tests and all are negative after 9 months. I believe this is really really conclusive but I just want to point out that I keep having recurrent throat infections and my doctors cant find the reason why. Do you think this could be by any chance something related to HIV?
2) How exactly does testing for Gono and Chlamydia works? Ive had 2 urethral swabs (1 at 2 months and 1 at 4 months post exposure) and 1 urine test at 6 months, all negative. My only exposure was oral sex (receiving) and vagina sex thats why I did this tests. I understand that these infections are only tested at the site of exposure. But considering my throat infections and that I also experienced at some point (still do occasionally) some rectal pain. Should I look for these in other sites like anus and throat?
3) I recently started having sex again with my girlfriend and I noticed something that was never there, after sex there seems to be this white thick substance resembling something like thick yogurt or cottage cheese around my penis, it seems to be coming from here. Thats why I as about tests for Gono and Chlamydia, maybe ive been doing it wrong? What does this sound to for you? Its a strange discharge Ive never seen
Thanks

Welcome back, but I'm sorry you found it necessary. I reviewed your two discussions with Dr. Hook. He made it very clear that your symptoms are not possibly due to HIV or any other STD, and I agree.
---
1) Many people get prolonged sore throats for various reasons. Allergy, some sort of irritation, and certain harmless infections might do it. And many cases are psychological, i.e. anxiety raising awareness of minor symptoms that otherwise would not be bothersome. That your doctor can't name a cause does not even hint that it's HIV. Second, HIV would never cause sore throat and/or rectal pain as the only symptoms. But most important, your blood tests indeed are conclusive, and so are the syphilis test. Your sore throat has absolutely no relation to the sexual exposure on your mind, other than possibly being related to anxiety over that event.
2) Your rectal pain is not possibly due to gonorrhea or chlamydia. You are correct that only exposed sites can become infected, so it is proved you do not have urethral gonorrhea or chlamydia. A throat swab is the only way to know if you might have oral infection, but it woudl be a waste of time and money. Neither gonorrhea nor chlamydia causes prolonged sore throat, and both are cleared by the immune system (without treatment) long before 9 months. Still, if your throat hasn't been tested for them, you could ask your doctor about throat swab testing for reassurance.
3) You describe what sounds like normal vaginal discharge for some women, although in others it could indicate yeast infection. This sort of discharge does not suggest any STD. Speak with your partner about it: if she thinks nohting is wrong, you can rely on that. If she isn't certain her discharge is normal, she should speak with her doctor, especially about possible yeast infection.
Please do your best to believe the science and the advice you have had. Try to put the exposure 9 months ago out of your mind. You have no infection from it.
Best wishes-- HHH, MD

Thank you doctor for your answers.
1) I understand my tests are way beyond the window period and are very conclusive, but recurring symptoms dont let me move on. I also feel very weak and tired most of my days and specially in my legs and arms, sometimes even feel like I cant pull up my arm. Does this indicate any kind of infection?
2) If you could explain how exactly could Gono or Chlmaydia get into my throat? I ask because of your statement about testing via throat swab for reassurance. Just to clarify my exposure was with a street sex worker and it was fully protected (both oral and vaginal sex), the girl gave the condom to me so I don't really know what brand or anything, but I used it the whole encounter and I did NO perform oral sex on her, I only received oral sex and had vaginal sex. Is throat infection possible in any way?
3) If by any chance I had gono or chlamydia, can my body cleared me of this by itself with some time? You say long before 9 months, is this for throat infection or is it also for other sites?
4) This thick white substance is very coincidence and thats my main concern. We've been together for some years and Ive never seen that with her till now. She says she doesnt feel any pain or itching on her vagina. Do you think this is something to be worry about?
5) I wouldnt say sore throat and rectal pain are my only symptoms, Ive had a lot of symptoms these months, 1-2 months after the encounter I found myself with weight loss (around 10 pounds) and lot of diahrrea and such.. Sore throat (recurrent tonsilitis) and this feeling of weakness are the most persistent symptoms to this day. Also my tonsils have this white patches on them and white dots that really don't go away, they have been there for months now. Ive had few throat cultures and nothing comes from that. What would you think could be causing this? Ive also been diagnosed with chronic gastritis
6) Sorry but all the details but I just want you to know everything so you can give me a direct advice. With all these HIV tests and all the symptoms I've told you about, do you really believe there's nothing else to do? I'm absolutely on the clear, lets say if I never have sex again ever in my life (of course hypothetical) I wouldn't need to test for HIV ever again?

Except for question 2, these are repetative questions whose answers are obvious either from my comments above or Dr. Hook's.
2) Apologies! I now see I misunderstood your exposure -- the oral sex was receiving only, i.e. you didn't perform oral on your partner 9 months ago. Therefore you cannot have oral infection and should not be tested for it. Sorry I misunderstood and for any temporary confusion about it.
3) There is no chance you had either one -- but if somehow you did, the answer is yes: the immune systems clears almost all gonorrhea or chlamydia before 9 months (in males; it can take longer in women).
4) I have no other comments about this. See my reply above.
6) Yes, I stand by my advice: there is nothing more you should do in regard to the sexual exposure you are so obsessed about. I remain confident your problems are unrelated to it except possibly for the emotional connection.
Please no more questions that have already been answered. Got it?

Thanks for you answers, its been very helpful. I just have a few follow ups to try to put my mind at ease.
When you search online (terrible idea, I know) for my symptoms, most of all if not all state that HIV infection could be the cause. Which makes me think that HIV is a very widespread disease and I dont really understand how you could point out some symptoms as not to be from HIV if the disease could probably cause all of them. For example Ive been having some pain and numbness in my thighs and recently also in my arms, feeling of weakness and numbness. When I look it up, one of the reasons could be HIV and that the disease could damage nerves. When I add this to all my other symptoms like weight loss, night sweats and more, I hope you understand its pretty difficult to look the other way. Specially when Ive been trying to figure things out with my doctor and none could give an answer on what is going on and why Ive been having all these symptoms. 1) My question is, if you could explain, how in this setting could you consider HIV totally out of the question? You really dont worry about possible HIV 2, possible false negatives, seronegative patients, possible effects on steroids and testing and biotin and testing and all of this "exceptions"?
2) What is the difference between "HIV-1 RNA (NAT)" test and the "HIV-1 Real Time PCR" test? My local lab offer both of them , the first cheaper than the second. I was thinking on taking one at 12 months post exposure but not sure about the difference and which one should I take
3) I took both these tests near 6 months post exposure and both were undetectable. Is it possible to test this far out and be undetectable but have the virus? I've read that some people suppress the viral load below detectable levels at 6 months or more. What could make this happen?
4) During one of my urethral swabs, my doctor did a DRE exam to get prostatic fluids into the sample, he then told me my prostate was inflamed and I had prostatitis. Is it possible that some bacteria passed my urethra and got into my prostate and that's why my urethral swabs and urine test came back negative but I indeed am infected? I was treated with Bactrim for 15 days because of the prostatitis. This didn't do much for my symptoms.
5) Is there any other STI or infection I should be looking into that may be causing my weak urine stream, my thigh pains, my groin pain and all the other?

Sorry forgot one question
6) Are my 9 months RPR and FTA-ABS tests conclusive? Even tho Ive had no symptoms (ulcer that I know of or rashes)
When I say 9 months in my questions is actually 5 days prior to actual 9 months, but I believe theres no difference, am I right?

1) HIV can be responsible for a giant number of symptoms. Indeed, probably the majority of all non-traumatic symptoms humans experience from anything can occur with HIV. Because they occur in innumerable other conditions, their presence usually does not indicate HIV. HIV is "out of the question" in your case because the test results are conclusive, and ALWAYS overrule symptoms, no matter what they are. Period, full stop; there has NEVER been an exception.
---
---
---
2) Those two tests are the same thing -- perhaps different test manufacturers, but basically the same test.
3) No, not possible.
4) Prostate infections are complex, but they commonly are isolated to the prostate itself and do not involve the urethra. Nothing unusual here, and nothing that implies either HIV or other STDs.
5) There are no other STDs for you to be tested for.
6) Your syphilis blood tests also are conclusive -- any time more than 6-12 weeks after exposure.
It is not normal to be so resistant to the repeated, reasoned, science-based advice you have had about HIV and other STDs, probably from your own doctor(s) in addition to this forum. You clearly are irrationally obsessed, and it seems to be having a significant negative impact on your life. You should consider professional counseling about it, which I suggest from compassion not criticism.
However, this forum is not a substitute for that. The forum does not permit repeated questions on the same topic or exposure. This being your third about this exposure and your unwarranted concerns about HIV and other STDs, it will have to be your last one; future ones will be deleted without reply and without refund of the posting fee. This policy is based on compassion, not criticism, and is designed to reduce temptations to keep paying for questions with obvious answers; because experience shows that continued answers tend to prolong users' anxieties; and because such questions have little educational value for other users, one of the forum's main purposes. I trust you will understand.
Best wishes and stay safe.
