[Question #9527] STI Risk Assessment

Hi Dr's

I am a mid 30's male in a Major Australian City.

Approximately four years ago I entered a mutually exclusive relationship. But before this I had a lot of partners. Including girlfriends, sex workers (both female and transexualls with penis) and had not always used condoms with previous girl friends and a couple hook ups with friends. I always used a condom for receptive anal sex and any sex with transexuals and maybe sucked on a penis once for a bout 30 seconds without a condom (about 5- 6 years ago) all of at registered brothels. I did however give cunnilingus unprotected very regularly to women, including girlfriends and sex workers.

After this I had STI tests for Chlamydia, Gonorrhea, Syphillis, HIV and Hepatitis A and B (All negative). It had never occurred to me to be tested for rectal or pharyngeal Chlamydia and Ghonnorea.

Approximately 3.5 years ago I received unprotected oral at a massage parlor and did the same battery of tests again which were all negative.

Following this approximately 20 months ago I received a blowjob which was protected from a transexual (anatomical male) at a brothel. When I took the condom off (It was intact) I then touched by penis head and washed it.

I then had a urinalysis due to frequent urination overnight (I think it was to drinking too much water) and they said I had trace Hematuria but no signs of infection and that microscopic urinalysis was completed as well and found no bacteria.

I have had a couple handjobs from transexuals and they touched themselves briefly and touched themselves whilst touching me.

I have since had a dipstick test done again with the same results trace hematuria.

My partner and I both had antibiotics due throat infections (unrelated as children were sick too) she had amoxycillin for 6 days and I had keflex 500mg x 3 times daily and roxyrithromyocin 150mg x 2 daily for 5 days (antibiotics overlapped by 3 days). 11 weeks after this she had a cervical swab as she is pregnant for Chlamydia, Ghonnorea, Trich

Trich and a couple other things which were all negative. We would have had sex approximately 9 times after her antibiotics.

The reason I am concerned is due to the microscopic hematuria as I had read online it can sometimes be cased by an STI. I have no other symptoms. I had some redness at the uretheral opening for a bit, but I think was due to excessive inspections and washing.

1. What are the chances that I have chlamydia or ghonnorea urogenitally based off the above information? Do I need to be tested?

2. Would the antibiotics I took have killed chlamydia or ghonnorea if I had it? At all possible sites of infection.

3. Would my partner have tested positive If I had it?

Welcome to our Forum and thanks for your questions.  I'll be glad to comment.  Your tests prove that you were not infected and the activities you report since your last test did not put you at risk for any STI.  The information you received (I presume this was from the internet where much of what is said is incorrect) is wrong. Hematuria is not a sign of STI.

In response to your specific questions:
1. What are the chances that I have chlamydia or ghonnorea urogenitally based off the above information? Do I need to be tested?
Your chances of having acquired gonorrhea from the casual contacts you describe is virtually zero.  Nothing you describe suggests a need for testing

2. Would the antibiotics I took have killed chlamydia or ghonnorea if I had it? At all possible sites of infection.
The antibiotics you took are not the recommended treatment for gonorrhea.  They would be expected to cure some but not all cases of gonorrhea.

3. Would my partner have tested positive If I had it?
Most probably.

I would advise you not to worry further that you might have been infected by your past encounters.  EWH

Hi Dr Hook,
 Thankyou for your reply.

You mentioned ghonnorea but not Chlamydia. What are the chances of acquiring chlamydia from the contacts required? Is it virtually Zero as well? Are there any othe STI's I should be worried about?

I was never tested for Pharyngeal or Rectal Chlamydia or Ghonnorea. Do I need to be tested for these?  I had never thought to suggest testing for these?

Correct.  Risk of gonorrhea and other STIs is likewise virtually zero.

No concern about pharyngeal or rectal STIs.  Given your recent antibiotics and the nature of your contacts, this is not something to worry about.

This completes this thread.  Take care.  Please don't worry.  EWH

I didn't mention that I did give protected blowjobs about 7-8 times about 6 years ago and just wanted to check that this doesn't change anything regarding your responses?
