[Question #9551] Please help, anxiety and stress HIV

Hello Dr. Im a 35 yes old male who is having some weird symptoms. Lower abdominal pain bloating and lots of growling, diarrhea for the past 3 weeks, white coated tongue, slight tenderness in left armpit lymph node, slight sore throat. I tested negative on a RNA HIV test (from veins) in day 35. I’m testing again duo 4th gen ag/ab and another RNA yesterday; day 46. I’m truly panicking because my life will be ruin if the test come back positive.
I made a stupid mistake with a stripper (don’t know her status; though I know she uses drugs) we end up in the private room where she danced naked, and things got heated and I stupidly pull my privates out and she dance on top of it rubbing it while she was wet. Then soon after, while she was dancing some clear liquid with a gooey substance squirted out in my private. I was upset and we stopped there. I couldn’t clean myself right away; plus I’m uncircumcised. Then I’ve started feeling weird symptoms afterwards like burning urinating, pain in testicles, UTI that took me to the ER, flank pain, pelvic pain etc. I was prescribed multiple antibiotics and the only one that seems to work was Azithromazin with Rociphen. The symptoms in the first sentence is was left of it after 3 weeks of antibiotics. Also, I’ve had multiple std test (urine) I have heard antibiotics can affect the urine test. The only test I took without antibiotics was 7 post exposure and it came back negative.
Like an idiot I had sex with the wife a day after the incident and now she is feeling sick. I’m truly scared I pass something to her though she tested negative for Chlamydia, Gonorrhea and Trich. Could it be HIV doctor?

I'm happy to reassure you: there is no possibility you have HIV. Your negative RNA test at 35 days is 99+% proof you did not acquire HIV, and it is 100% proof that HIV isn't the cause of your symptoms. It is not possible to have HIV symptoms if the RNA test is negative; that result overrules even the most typical symptoms of acute retroviral syndrome (ARS, i.e. initial HIV infection). And your symptoms are not typical for ARS anyway. Also, you had a zero risk exposure in regard to HIV, even if the dancer has HIV -- which is statistically unlikely. (The "squirt" almost certainly was urine, which almost never transmits STDs and never HIV. But even if it was vaginal fluid and she has HIV, there was no risk you could be infected by such an exposure.
And almost certainly you had no other STD either. I wonder if the ER make a confirmed diagnosis of UTI on the basis of urinalysis and urine culture? Or was that based only on your symptoms? I'm suspicious about that diagnosis. But in any case, UTI is not an STD. That your symptoms improved with ceftriaxone and azithromycin may have been a coincidence, but doesn't mean you had an STD. Unfortunately, if the ER didn't do a urine gonorrhea/chlamydia test, you'll never be able to know for sure if you had either of these STDs. Your negative test after treatment means only you weren't infected at the time of the test; if you were infected before treatment, it was cured. But for the reasons above, I strongly doubt you had any STD. And in any case, your wife's negative tests show you did not infect her.
So you definitely can expect your pending AgAb (4th generation) and repeat HIV RNA test to be negative. But even now, your HIV worries should be over. There is no chance you have it.
Let me know if anything isn't clear. But maybe best to wait until you have the repeat HIV test results, if you would like my input about them. Stay mellow in the meantime -- all is well!

Thank you doctor for your speedy response.
To answer your question about UTI, yes, I believe they diagnosed UTI in the ER with urine test not culture. I also saw my primary care regarding the diarrhea and abdominal tenderness and sensitivity. It’s been 3 weeks with constant loose stool watery, yellow and with lots of flatulence when going to the restroom. He did a stool sample to check for any bacteria; if none, then he will refer me to a GI specialist. I’ve also stayed away from any antibiotics to make sure this HIV and STD panel are conclusive for my mental health. I got a few STD PCR std tests done:
Urgent Care clinics - one 7 days post incident (no antibiotics), 12 days (was taking doxy/Rociphen).
ER - 16 days (taking Cipro, and finishing doxy).
All of them have come back negative. During my last ER visit, the only diagnostic besides the UTI has been mild colitis. I’m also seen a Urologist who has done multiple test like Cytoscopy, ultrasound in bladder and other organs besides the stomach and blood test….everything has come back normal. The reason i keep panicking is because my wife is having strange symptoms after having relations with me: UTI, tiredness, itchy skin, joint pain, diarrhea, headaches, dry cough, chills and night sweat (two nights), and possible fever though she didn’t take her temp. Not everything at the same time but, one after another.
I truly appreciate you and god bless you for keeping all of us in a semi normal state of mentality. I was considering ending my life to say the least, before the PCR test result. Not the best state of mind. I have three kids and they are the world to me and if I I lose that, there is no future for me. Again thank you for your reassurance and your quick response. I’ll let you know the results once it comes back.

Thanks for all the additional details. Having read them carefully, they do not change my bottom line evaluation and advice. There is no STD that causes the multiple symptoms you describe; if due to an infection of any kind, it probably is unrelated to the strip club events described; or if it is, you acquired a non-STD virus while there. Your wife's symptoms also are not consistent with any STD, but it's possible she acquired your likely viral infection.

While I understand your concerns, they are out of proportion to the risk of STD or HIV, or of any other health consequence of your strip club visit. You and your wife should keep working with your doctor(s) if your symptoms continue or you remain concerned. But I am confident you can dismiss any possibility of HIV or any other STD.
I'll be happy to make a final comment or two if you'd like to let me know your follow-up HIV test results, but will have nothing more to say until then.

Dr, I just received my results. As you predicted my HIV ab/ag & RNA blood test from veins came back negative at 6 weeks/46 days. I also tested for other STDs Hep A/B/C, Chlamydia, Gonorrhea, Herpes 1 and 2 came back negative. I’m still having daily GI issues like the one I explained in the previous questions and yesterday a new symptom appeared randomly burning pain after ejaculation (self masturbation). I feel my urethra sore and my penis head gets pretty red after masturbating. I’m sure you would not recommend masturbation at this time but, I just needed to test the waters and see if my privates felt normal after 6 weeks of hell and now I got myself this weird symptoms. Now, since I can finally discard HIV (which is what I’m scared of the MOST) what’s do you recommend me asking other specialists.
I have an appointment this week with an Infection Control Specialist, an initial appointment with GI specialist for possible colonoscopy and a Urologist appointment for a Cystoscopy.
Do you think I should ask the IC and/ or Urologist specialist for a swab test in my urethra, throat and anus and semen culture in case the urine test missed something? From your experience what other question should be beneficial in my case. I hate wasting a specialist time since I know you guys time is so precious.

Of course your HIV tests were negative; there was no other possibilit since it was already proved you do not have HIV.
We do not attempt to judge the cause of symptoms one HIV and other STIs have been ruled out. Frankly, none of your symptoms sounds at all serious and I'm not sure any further medical evaluation is necessary. I would advise not requesting any particular test from any specialists you see -- let them make their decisions without interference from your non-professional input. Of course it's OK to ask if any particular tests or procedures are worth considering, but I see no possible value in the ones you mention.
