[Question #9572] Follow up Q: [Question #9554] Risk assessment after unprotected oral

Dear Drs. This is a follow-up question to one I asked a few days ago - I should maybe not have closed it so rapidly. To recap: I had sex with an escort 2 weeks ago, protected vaginal (condom) and unprotected oral. No symptoms following that; I then had unprotected vaginal sex with my regular partner 5 days later. The day after, she developed symptoms she describes as a UTI, from what I can glean this is some urinary urgency and pain on urination.
These symptoms continue and she does not want to go to the GP, preferring to try and let it resolve naturally. I had very slight meatitis at the time I asked the question, which continues to today. Interestingly I have just yesterday had teeth extracted and am taking amoxycillin, which seems to have stopped the meatitis - suggesting maybe something like e.coli was causing it? I have not at any time had discharge.
Dr Handsfield's advice was that given the protected vaginal sex and unprotected oral, and the immediate onset of UTI symptoms in my partner, the only reasonably likely STD would be gonorrhea, however my lack of discharge or pain on urination make that unlikely.
My partner's ongoing symptoms now at 1 week are worrying me. I bought some urinalysis test strips from the pharmacy and both yesterday and first thing this morning (so 8+ hours after last urination) was negative for leukocytes and nitrites. This morning's test was possibly very lightly positive for protein. I appreciate these are not conclusive tests but maybe provide more evidence.
If I was living in the UK where I'm from, I would go to a sexual health clinic and get tested, unfortunately I live in Switzerland. While there are some clinics here I cannot get an appointment for a test until February, which seems absurd; I could go and see my GP but it will take time to get an appointment and also it's the same GP as my partner which I feel uneasy about.

I guess I am just looking for reassurance that, especially given my negative urine dipstick test, it remains unlikely that I have an STI.

Welcome back to the Forum. On this occasion I happened to pick up your question and will be commenting. In preparation for my reply I reviewed your earlier interaction with Dr. Handsfield and agree with all that he said. I’m sorry your partner’s symptoms continue. As Dr Handsfield stated, development of UTIs in women following intercourse is not unusual and your partner’s rapid onset of symptoms is far more consistent with a UTI than an STI. Typically the onset of STI symptoms takes at least several days. I think testing would be reassuring for you but that for your partner testing is important. Untreated UTIs can lead to more complicated infections. The UTI test strips are sometimes helpful but are not perfect.
Bottom line, your risk for STI was low but not zero, particularly if you have persistent meatitis. It is worthwhile for you to be tested. It is more important however that your partner be evaluated.
