Good Evening Doctors. 

I will start off by saying this has caused me a great deal of stress and anxiety.  

12-21-2022 I received a condom protected blowjob from a female brothel worker in the dominican Republic.  She told me she was negative for all stds, as a requirement for employment.   She made sure her lips never touched skin.   And she used a towelette of some sort to wipe condom before. 

1-15-23 (25th day post exposure)  evening, I get throat tickle, which brings on a dry cough (occasionally) getting brownish phlegm out.   Also had a fever the following day in the 101 range.  Today, about 48 hours later, I feel much better.   Still low grade fever on and off up to 99.9 orally.     I am in a dorm setting, and other people also seem to be sick with the same, but have not reported fever because they don't have a thermometer.   But reported chills, body ache and waking up to wet beds.   

Logic tells me that I caught what everyone else has.   Anxiety tells me that my fever/dry cough/head aches (from dry coughing) are ARS.   Due to the timing.   Also, loose stools today that I guess attribute to dayquil and Anxiety.   

Only really worried about HIV. 
Plan to test at 6 weeks. 

Am I overthinking this?

Going to your closing question:  Yes, you are overthinking this.

First, you describe a risk free exposure. Even without condoms, HIV is not transmitted mouth to penis, or so rarely there has never been a reliably documented case. And condoms are 100% protective as well. Second, you misunderstand acute HIV symptoms; they are typical for a garden variety respiratory virus (including Covid), but not HIV. The symptoms of acute retroviral syndrome (ARS, i.e. initial HIV) do not include cough or sputum (phlegm) from the chest. In addition, ARS symptoms cannot start more than 3 weeks after infection; they always begin within 8-14 days. In other words, your "logic" is exactly right.

You do not need HIV testing, except for the likelihood that the negative result will help calm your fears. You definitely can expect a negative result. You really don't need to wait until 6 weeks after the event. A negative result now will prove your symptoms are not due to HIV. (It isn't possible to have HIV symptoms with a negative blood test.)

Do your best to stop worrying. This isn't HIV. Let me know if anything isn't clear.

I am an extremely anxious person.   After your last comment I woke up that morning to a sweaty pillow.  Fever gone   So my fever lasted 48-60 hours.   Guessing that's too short for ARS fever?   Just have lingering dry cough. 

I slept with my wife yesterday, I feel guilty for stepping out of my marriage.   Should feel confident that I didn't put my wife at risk of anything? 

I would test now, but I already know my anxiety will want a conclusive result.  So I'm guessing 45 days.

Sorry, I can't help the underlying anxiety. You do not have HIV. Sweating happens when fever drops; probably your pillow was damp because your fever cleared. This happens regardless of the cause of the fever. You do not have HIV and have not put your wife at risk of anything.

But if you insist on testing to help relieve your anxiety, you do not need to wait until 45 days. An AgAb test ("4th generation") is usually positive at 3-4 weeks, so a negative result then will be 98% conclusive. You can then have a second test at 6 weeks (or 45 days) if you feel you need a still more conclusive result.

Do your best to separate your regret and shame over a sexual decision you regret from health risk from that event. They aren't the same.

As you said Doctor, I was not at risk.   My 43 day STD 10 panel test was completely negative / non reactive.  

My HIV test was a 4th generation done by clinical pathology laboratories.   

The syphilis test was "3502 RPR REFLEX TO T. PALLIDUM - PA"

Are all std tests conclusive by 43 days?  

A million thanks.  I am so sorry for letting my anxiety get away from me.  On the hard days I would reread our thread, it kept me going.    Thank You So Very Much.

Yes, you can rely on those results. Some experts advise 3 months for RPR, but the combination of no symptoms (a sore at a site of skin contact) within 3 weeks plus negative RPR at 43 days is conclusive.
