[Question #9631] Unprotected oral csw hsv1 hsv 2 concerns!!

Thanks in advance for your help. I’m a married 50 year old man and am worried I will give my wife something. I’ve seen several csw over the last year. I had unprotected vaginal sex and unprotected oral with one girl Aug and Sept and last time in early October 2022. I know this is so stupid and dangerous. I also performed oral on her same girl. I did not see any lesions on mouth or vagina. With anxiety I did a 10 panel test several times including trich after.
Dates for test
8/25/22, all negative
9/26/22 all negative
11/15/22 all negative
All hsv1 hsv 2 .090
The following events have been unprotected oral and protected condom with vaginal sex. 12/23 and 1/3. With some rubbing my penis on her clit for maybe 1 minute no penetration. Three days later I started feeling tingling type sensation on penis. The normal bumps and variations are more pronounced and are Irritated kind of redish NO Blisters. Glans feel kinda sticky and cold to the touch at times with slight discomfort with urination and frequent urination and a kinda of discomfort in glan like mild burning on and off to this day. I have also noticed some red irritation kinda bumps on butt crack with some itching but it’s hard to see. I do also have frequent ibs so that may be related. My following test are 10 panel on 1/9/23 all neg but VSH1 pos 3.1
VSH2 .090 negative
Next two test just vsh1 and 2
1/16/23 Vsh 1 2.30
VSh 2 neg
1/27/23 vsh 1 1.90
Vsh 2 neg.
My wife has also said she is feeling irritation on her vagina like burning sensation. . She did have a root Canal and has been on antibiotics for two weeks it maybe a yeast infection but its freaking me out. Do you think this is a new Hsv 1 genital infection? Could it be a prostate issue or yeast infectisomething along those lines . I’m having serious anxiety and guilt I’m having trouble sleeping which I know isn’t helping. I’m Done with this stupid mid life crisis. Also I don’t recall having cold sores on mouth. My wife has had some over the years.

Have you ever in your life had a cold sore in your nose perhaps? If yes, that could be the cause of HSV 1- common site for outbreaks of HSV 1. And if your wife has had them over the years, then she has HSV 1 and may well have given them to you somewhere on your body. In other words, she has the thing already that you are positive for so even if you did acquire HSV 1 from a sex worker, you won't give her again what she already has. is that helpful?

Thanks for the reply. I haven’t had a cold sore on my nose or face, my wife rarely gets one on her lips maybe 1 time a year if at all. I guess my questions are.
1. Do you think my symptoms on my penise head are herpes related?I haven’t had any blisters just what I described in first message.
2. Do you think my hsv1 test are correct they changed from negative to positive? Can I rule out hsv2?
3. My neg results from tests on 1/9, 1/16 and 1/27 for everything else would be conclusive? Meaning HIV, Syphilis, gonorrhea, chlamydia. My stupid uncovered events worry me a lot.
4. If not a herpes issue what other causes could this be? Could it be bacterial balanitis? My main syptoms are frequent urination and discomfort/ irritation Inflamed redish raised bumps pain is not bad maybe a 2 out if 10.
5. Can this be from my elevated anxiety?
On a side note I am on a antifungal medication for a toenail issue terbinafine 250 mg. Would this rule out a yeast type infection?

I did reply with a group of responses 2 days ago but think that the internet is unreliable here in the jungle to push everything through. Sorry about that.
1. I do not think they are related at all, no
2. I can't tell if your HSV 1 result is accurate or not - do you have the index value of the test? If yes, that would be very helpful
3. Yes, you can count on those for very accurate timing
4. They could be balanitis for sure, more likely fungal but I note you are taking antiviral meds and they should be helpful if you have fungal balanitis but that's not a 100% sure thing - a topical antifungal might be helpful to try.
5. I don't think people get symptoms from elevated anxiety BUT I do think people can notice very small changes in their body that may never have been noted had there not been an experience for concern
If you do have HSV 1, this could easily have come from your wife and not anyone else, right? If you want a clearer answer about your HSV 1 status, you could do the herpes western blot for greatest accuracy, but what would you do with the results that is different than what you are doing now? The fact that your wife has HSV 1 makes all of this situation much less troubling, I think.

My hsv1 test are as follows
HSV1 Neg .90 on 8/25, 9/26, 11/15
HSV2 neg .90
Fast forward to a unprotected oral event with some rubbing my penis on her vagina NO penetration at all. Date 1/3
All test are as follows
HSV1
1/9 hsv1 3.1
1/16 hsv1 2.30
1/27 hsv1 1.90
All hsv2 test have remained neg .90 on ever test.
There’s definitely something that’s going on with my penis glan. I do plan on seeing a urologist, I’ll have him look at it and also get a answer for the frequent urination and cold feeling down there. The kinda discomfort/mild irrational burning feeling has subsided.
My plan is to wait a full 13 weeks from exposer and retest to make sure the Hsv2 especially didn’t change. With that result being neg hopefully I’m going to put all this behind me, and by then I’ll have s answer to my symptoms.

So here's the thing about all of this: even if this person did have HSV 1 genitally, which isn't very common, HSV 1 genital infection is rarely shed from the genital tract - after being infected for two years, people shed about 4 days out of an entire year. The likelihood that this would happen at the moment that she was rubbing your penis on her vagina for one minute seems so unlikely.
The only thing I can think that MIGHT have happened with the CSW would be that you got infected with HSV 1 from receiving oral sex in October and it didn't show up until the January test, But it is also possible that this is a false positive AND it is possible that you acquired HSV 1 from your wife during that time. If you test negative for HSV 2 at 13 weeks, I think you can count on that one. If you are actually positive for HSV 1, without any sores anywhere, it will never be possible to know for sure where it came from or where the infection is.
If you do more testing, be certain they are all done at the same lab so you are comparing "apples to apples".

At this time a may have one more question, also this may help people that may have similar issues. Thanks again for all that you and the other Doctors do to help people! The information you get online is very misleading and troublesome. This website really is a wealth of information that can be trusted by true professionals.

I'm very glad that you find our service helpful. Remember that there are also false positives on the HSV 1 test so if this next one is also positive, below 3.0, it may not be accurate and a western blot would clarify. I'm afraid you are out of questions at this time. But if you have more, you can do a new subscription.
