Hi doc ,
Sorry to bother you again but wanted to check a few more things :
1. I am confused about what foods or meds interact with the efficacy of pep . I am currently having whey protein and i read th ingredients , it also contains digestive enzymes and ashwagandha (indian genseng). Do any of these interact with PEP?
2: I am not sure if daily vitamins would affect pep and hence stopped them .
3. Any specific protocols to be followed while taking pep ?
Request you to please advise . Thanks

Welcome back to the Forum and thanks for your questions although I'm sorry that you felt the need. Straight to your follow-ups which, to be honest, are better addressed to your pharmacist than me:

This question is better addressed to your pharmacist. In general medications are better absorbed if taken an hour before or two hours after a meal although some medications aare actually recommended to be taken with food. I know of no data to suggest that ginseng interferes in any way with anti-HIV medications (or for that matter, tests for HIV)
2: I am not sure if daily vitamins would affect pep and hence stopped them .
No need to stop daily multivitamins taken at normal doses
3. Any specific protocols to be followed while taking pep ?
Follow the instructions from your pharmacist
Hope this is helpful. EWH

Hi dr ,
Unfortunately in india pharmacists don’t give much advise on protocols . Hence i request if you could please help me .
1. Would added digestive enzymes in whey protein affect the efficacy of pep ?
2. I even read a few articles saying garlic can affect pep efficacy . Is this true ?
3. Do i need to do a test after completing 28 days of pep ? If yes after how long . Request your help
please .
4. I read a few websites and they said fingering and kissing if a small cut is present is a low risk or not high risk . That got me a bit worried . Request you advise please . Thanks

1. Would added digestive enzymes in whey protein affect the efficacy of pep ?
No, not if taken as directed
2. I even read a few articles saying garlic can affect pep efficacy . Is this true ?
No.
3. Do i need to do a test after completing 28 days of pep ? If yes after how long . Request your help
please .
You did not need PEP in the 1rst place as I told you. For persons who take PEP, follow-up testing for HIV with a 4th generation, combination HIV antigen
antibody test at 6 weeks after completion of PEP is recommended
4. I read a few websites and they said fingering and kissing if a small cut is present is a low risk or not high risk . That got me a bit worried . Request you advise please .
As I said, this was no risk.
You need to relax. I also advise you to stay of the internet. Much of what is there is incorrect and/or misleading. EWH

Thanks dr,
Also today i noticed pain on the back of my left ear lobe . i noticed a tiny bump out it’s kind of pinchable , i’m not sure if it’s an abscess coz
if swearing or or it a node . Would a single node behind the ear come up 5 days post exposure ? I mean if the perosn is on PEP is it possible for a node to come in 5 days ?
Or can i forget this and move ahead . Request you to please advise . Thanks again

As you know we provide up to three responses to each client's questions. With this 3rd response this thread will be closed without further responses.
There are not lymph nodes at the ear lobe although there are lymph nodes which can sometimes be found under the base of the ear in the upper neck. The swelling you describe ins not only a location where lymph nodes do not occur but is also atypical. With or without PEP, an HIV-related lymph node would not appear just 5 days after an exposure.
This completes this thread. It is time for you to move forward without continuing concern. Please note that you have now participated in two sets of questions related to the no risk exposure you described. It is time for you to put your unwarranted fears aside and move forward without concern. Please do not return to the site with further anxiety-driven questions regarding the no risk events you have described. If you do, your question may be closed without a response and without return of your posting fee. Take care. Please do not worry. EWH
