December 7 th 2022 received an unprotected blow job from licensed Spa in Abu Dhabi , Blow job given by trans person initially I mistaken as women . Due to fear after 11 day did RNA Pcr test from Better2know and which is negative then after 45 I did hiv combo test ( including p24) from 2 labs one from Better2know and the index values is .08 for both
The symptoms I noticed is 15 days after neck stiffness and Razor bumps / pimples on the face ( which is generally. I used to have after shaving)
After 45 days again more pimples on my face
My question
how reliable is hiv combo test after 45 days .?
Is pimples(red Color with white patches ( Dry skin due to cold weather) is symptom of HIV ?
Do i need an another test?
Ps:- generally I have skin problem with acne on head & shaving bumps on face this time due to fear I may be over noticing

Your worries are over: you were not at risk for HIV from this event and you do not have it. There has never been a scientifically documented case of HIV transmitted oral to genital. That doesn't prove it cannot happen, but it is exceedingly rare. Based on how people believe they were infected (probably often wrong), CDC estimates the risk of fellatio by an HIV infected partner at one chance in 20,000. That's equivalent to receiving BJs by infected partners once daily for 55 years before infection might be likely. A second factor is that according to a prominent STI/HIV clinic in Dubai, HIV is rare in female sex workers -- so the odds are strong that your partner doesn't have HIV. (If anatomically a male who has frequent sex with other men, the risk is higher, but it remains unlikely s/he has HIV.)
As for your symptoms, they do not hint at having HIV, which does not increase acne/pimple problems.
Finally, your AgAb ("combo", 4th generation) HIV blood test is conclusive and proves you do not have HIV.
For all those reasons, it is certain you do not have HIV; you weren't even at risk. All is well and there is no need for additional testing.

days test is recommended for conclusive result . Even from
CDC and BHIVA websites I understood 99% infections will be detected within 45 days . So Doctor as per you is 4th Generations test conclusive @45th Day ?

I already said "an anatomic male who has frequent sex with other men" of course is more likely to have HIV. But the level of risk at the time of exposure is irrelevant at this point: no matter how high the risk at the time, your HIV test result proves you were not infected. Your doctor is mistaken: 90 days was the window for older HIV tests, mostly no longer in use ("second generation" blood tests), but not for third generation antibody tests (8 weeks) or the AgAb (4th generation) tests, which definitely is 45 days. (CDC is being conservative in advising "only" 99% certainty at 45 days. I have never seen anyone who tested negative at 45 days who later turned out to have HIV, and neither has any other HIV expert I know.)

As noted above, you were at such low risk that you did not need testing anyway. You definitely do not have HIV. Believe it!

This is my first experience outside marriage I am full of Guilt feeling .
My logic says this is low risk and to believe my test results . But some time there is some Feeling which seeing only the words risk, 99% and thinking what if I am in that remaining 1 % . Last 55 days literally the worst period .

Undoubtedly you are right that your guilt/shame over a sexual decision you regret is affecting your ability to objectively analyze the risks and accept my reassurance. But you are not going to be the first person in the world known to acquire HIV by a kind of exposure that previously has not been proved to transmit HIV. You also will not be the first known exception to conclusive results of the AgAb test beyond 45 days.
