[Question #9679] Further testing

Hello doctors I asked a question here 7 months ago. A man attempted to penetrate my vagina without my consent 7 months ago and I started PEP as a result. I took it for 30 days throwing up 2 of those doses. I tested with RNA 13 days post completion of PEP and Ag/Ab 41 days post PEP and both came back negative. 

It’s now been 6 months post pep and I tested yesterday. Usually my results come back the morning after but they haven’t yet and it’s causing me concern. What are the chances my results have changed since 6 weeks?

Welcome back, but I'm sorry you found it necessary -- and surprised you did. Reviewing your previous two threads, it was obvious then that there was little or no chance you acquired HIV during the exposure you are concerned about, and I'm sorry my words then were not as reassuring as I intended. In any case, an extra few days in getting HIV test results does not imply it was positive and I am confident it will not be. But feel free to let me know the result when you have it.

Hi sorry I should’ve included this in the first message, but he actually admitted to lying and did penetrate me. He still refuses to get tested however. That’s actually why I decided to go get tested again. 

Now knowing there was indeed a risk what do you think about the pep, 2 missing doses, my negative 13 day post pep RNA test and negative 41 day post pep ag/ab test and now my late and resultless 6 month post pep test? What should I expect the results to be? Thank you.

Thanks for that clarification. I understand why that elevates your concern. However, it still remains exceedingly unlikely you acquired HIV; go back and carefully re-read my replies in your older threads for the details of that analysis.

The question immediately after yours (no. 9880) also asked about missing doses of PEP. There is little no chance that missing a few doses reduces its effectiveness in preventing HIV; and your negative test results at 13 and 41 days also are nearly 100% proof you were not infected.

I'll be happy to comment one more time if you would like to post your latest test result when you have it -- but not until then.

Sorry I know you want to wait for my result but as I’ve mentioned, it’s later than usual and hasn’t come yet. 

I’ve seen similar situations as mine answered on here where people test at 12 days (rna) and 40 days (ag/ab) post pep and you say they’re conclusive and don’t need further testing, why is my situation different? Why are my tests at 13 days (rna) and 41 days (ag/ab) post pep only NEARLY conclusive and not entirely conclusive like theirs? Do you think it’s likely my result will change? Sorry but the difference in your answer for me as well as late test results are making me very anxious right now.

Also if we want to get technical I got tested on the 42nd day post PEP. I finished it on June 21st 2022, my first day without it was June 22nd 2022. I got tested on August 2nd 2022 around noon. So 41 FULL days without PEP but tested on the 42nd day from my last dose of PEP if it makes a difference.

You're seriously over-interpreting things. One day makes no known difference in test performance or reliability. On innumerable threads we have stated that most experts agree the AgAb tests are conclusive 6 weeks after last dose of PEP, but some experts estimate 3 months. Precise data do not exist. In any case, you must not focus on the exact wording of every reply we make:  the words we use often depend on context.

In addition, we consider the overall odds. In your case, under one in a million chance you acquired HIV. PEP effectiveness, let's say around 90%. That makes the odds you have HIV, after PEP, one chance in 10 million. If we also assume your tests so far are "only" 99% conclusive, not 100%, your odds of having HIV become one chance in a billion. That's zero for all practical purposes, even though the test itself isn't quite conclusive.

I did not ask you to post your test result. It was an offer, to say you are welcome to do so if you wish. ("I'll be happy to comment one more time if you would like to post your latest test result when you have it.") We regularly make the same offer; it does not imply we expect a positive test result. (Among thousands of questions over 20 years in this and our preceding forum, not one person turned out to have caught HIV. You will not be the first!)

Threads normally are closed after two follow-up exchanges, and we're there. I'll leave this open a few more days, so you can post your pending test result if you decide to do so. However, any further comment other than that will result in closure of this thread without reply. OK?

Hello I got my results back negative. This test was taken over 6 months after completing PEP (probably 7months) is this test finally 100% conclusive? Can I move on? No further testing?

Great! Glad to hear this, but of course not surprised. Indeed, this result is conclusive, confirming your previously conclusive results! I hope you're able to move on without further worry about HIV.
