[Question #9741] Follow Up

Hi Doctor
This is a follow up question to my last question.
Reading through my question i felt I miscommunicated mainly due to my poor English.
I am concerned about a massage session which involved Naked genital to genital rubbing and then both myself and the masseur using the vaginal filled hands (both me and lady massages her finger) to give me a handjob, thus a possible contact between the vaginal fluid and penis.
I started PEP, had 2 dose but I decided to stop and threw away the medicine.
I wanted to re-ask this question because I thought my earlier question was not phrased correctly and there were previous no risk exposure which I am not really concerned about.
Can you do a risk assessment for me again and consider this as no risk situation. I wonder if I will regret the decision of throwing away the PEP.

This is a repetitive question. Your English is fine and your question was clear. As I said before - "The encounters you describe are classified as no risk encounters which do not require testing and certainly are not a reason to receive PEP. HIV is transmitted ONLY through penetrative sexual contact or injection of infectious materials deep into tissue. HIV becomes non- infectious upon exposure to the environment. Circumcision status makes no difference. Specifically HIV in NOT transmitted by contact with genital secretions transferred from person to person on the hands during masturbation. All major pubic health organizations, the CDC, the WHO, and NIH AGREE on this."
Therefore, please note:
HIV IS NOT TRANSMITTED WHEN A PERSON WITH THEIR OWN GENITAL SECRETIONS ON THEIR HANDS USES THEIR HANDS TO MASTURBATE A PARTNER.
This was a NO RISK exposure.
You do not need PEP. You do not even need testing. Please don't worry. EWH

1. Why do the CDC state in their official website "The only possible risk would be if body fluids from a person with HIV touch the mucous membranes or damaged tissue of someone without HIV. Mucous membranes are found inside the rectum, vagina, opening of the penis, and mouth. Damaged tissue could include cuts, sores, or open wounds." is the Penis head not a Mucous membrane? 2. I threw away the medicine but got it back and took the third dose today. I had the masseur tested for HIV RNA PCR and HIV Duo, should know the result in next 3-4 days. I am thinking if the result turns out to be negative i can stop taking the PEP. I am praying it does not come out positive as it will only elevate my elevated anxiety. 3. Am I wrong to worry that genital rubbing can also lead to involuntary penetration and possible exposure to the virus?

4. I have previous no risk exposure like simple handjob from other massuer in last 2 weeks. I got myself tested yesterday with HIV Duo and it came out negative but it would possibly does not include the recent 2 week exposure. Assuming i take PEP for 7 days and stop, then after how many day should i be testing to get a conclusive report for my peace of mind sake.
And doctor 1 suggestion: when i post my reply or query, i do get notification that my post is very long and I need to trim it down. And in the effort to trim it down, i made the error last time where it gets difficult to convey the concern properly. It happened to me this time as well.. it kept showing that my post is too long, so i have to trim my 3 points. and it was not too long either. Not everyone would be aware that they can put additional comments over their post- I was not aware of it till yesterday. Only a suggestion- not sure if its a system error or its set that way. Some of my long post are accepted but other shorter ones are not

This will be my final response. Should you return with further, argumentative, repetitive questions you thread may be deleted without a response and without return of your posting fee.
1.Genital rubbing and masturbation are safe sex. The head of the penis is NOT a mucous membrane. The CDC says this. These practices NO NOT meaningfully introduce infectious materteral into the mucosal surface of the urethra. No one has EVER been proven to become infected with HIV as a result of receipt of oral sex in the more than 40 years since the beginning of the pandemic. You do not even know that your partner had HIV and it is statistically unlikely that she was.
2. You can take PEP is you wish. There is no medical or scientific reason for you to do so however. When the masseuses' tests show that she does not have HIV, this will prove that you could not have gotten HIV from her. Only persons with positive tests transmit HIV.
3. Genital rubbing does not result in meaningful introduction of secretions into the penis

4. Testing for HIV would be conclusive if you had a meaningful exposure 6 weeks after expsoure. Some experts recommend testing at 12 weeks or 6 months but this is overly conservative.
End of thread. EWH
