Hi Dr,
Sorry to be back here, just wana turn the page.
I reported to you my negative HIV test result 8.5 weeks post possible exposure. It was a lab vein drawn blood test with 24h result, in Montreal, so should be 4th gen abag test, right? Meaning this should be conclusive?
Now my question is about delayed seroconversion which has been asked many times before. I just want to confirm it in relation to the med I'm taking.
I take 20mg weekly Methotrexate, which I believe is one of those 'potent' immunosuppressive drugs. My lymphs are always suppressed (1,000) - last CBC even at 900.
Can I consider my test conclusive? do I need to retest?
I am moving to Dubai in a month and don't want to be detained if something comes up there.

Thank you

Just to add, I take Methotrexate for Sarcoidosis, sort of an autoimmune disease.

Welcome back to our forum, although I’m surprised you felt the need. As you know, Dr. Handsfield and I share the forum because we have worked closely for nearly 40 years and our opinions never differ, although our verbal styles are somewhat different. In preparing to answer your questions by reviewed your earlier interaction with Dr. Handsfield regarding your virtually no risk encounter and agree with everything he has already said.

Low dose methotrexate in the doses you take is somewhat immune suppressing.  The effect is best studied in persons who have recently received the COVID-19 vaccine and these studies show that persons taking methotrexate DO respond to the vaccine but that, in some, antibody levels are somewhat lower than in persons not taking methotrexate.  For you, the virtually no risk nature of your encounter, the relatively long interval since the encounter, and the fact that tests for HIV detect not only antibodies but the virus in the form of the p24 antigen as well combine to conclusively prove that you were not infected.

The idea of delayed seroconversion is an internet promoted myth.  You can believe your test results.

Please don’t worry. EWH
