[Question #9755] Risk from Massage

Hi doctor, I had an exposure 2 weeks back which involved naked body to body massage, genital rubbing, fingering, mutual masturbation with genital secretion involved. I initially started PEP and had the lady test for HIV- PCR and DUO and it came out both negative. I later stopped the PEP medication since she was negative. What is the risk from this exposure? Could she be in her Window period and still transmit the virus. Also the PCR test doesn’t capture HIV-2 probability. Also when should be testing for my conclusive result considering I have taken couple of doses of PEP. Also if I have to take PCR test, how early it could be

Welcome. Thanks for your question.
Occasionally the title of a question contains almost enough information for a helpful reply. Before I even read the question itself, I'll say that massage never is a risk for transmission of HIV or any STD, assuming no "happy ending" by oral sex. Even masturbation (hand-genital contact) or vaginal or anal fingering carry little or no risk.
Now I have read the body of your question. Guess what? There was no measurable risk of HIV. Had you asked before now, or had you come to my clinic, we would have not prescribed PEP, no matter how urgently you requested it. In any case, you did exactly the right thing in arranging for your partner to be tested, and her negative PCR and AgAb ("duo") test conclusively showed there was no risk, and I'm glad to see you stopped PEP at that point. As for HIV2, it is virtually absent in the US and most other geographic areas, other than parts of West Africa; and your partner's negative result in fact is conclusive for HIV2, unless she was exposed within a few weeks prior to her test, which is statistically almost impossible.
In these circumstances, from a medical/risk perspective, you do not need HIV testing at all. I would have said the same thing even if your partner had not been tested: even if she had HIV, there was no risk from the events you describe. However, I understand that some people are more reassured by negative test results than by professional opinion based on probability and statistics, no matter how expert that opinion. (We don't take it personally!) If you decide to be tested strictly for reassurance, I would advise only an AgAb test 4-6 weeks after the massage.

Hi Doctor, Many thanks for your reply. You guys are doing an awesome job.
1. I am from India. I think HIV2 is still rare in India less than around 3% of the cases are HIV-2. If she is in her Window period, would she be able to infect me with HIV-2?
2. The reason i asked for PCR test is reduce my anxiety. I have been thinking about this exposure for 2 week now and making me mentally sick and want to get over with and move on. So I ask the repetitive question (sorry) if I have to test for PCR for a conclusive report, how early can I do it. Please consider the fact that I have consume 3 days of PEP medicine.
3. I have seen other questions where you and Dr.Hook frequently says genital rubbing and mutual masturbation even with genital secretion is No risk activity. May be because of my anxiety, I think this was high risk exposure (I am not an expert and it’s my mind) because the chances of vaginal fluid exposed to Penis is high. How this is no risk scenario? Please explain
4. Can an Ag/Ab test at 4/6 weeks be conclusive for my case (3 doses of PEP)
5. This is really stupid question and anxiety one again. My 6 month old yesterday accidentally hit his hand against my teeth. There was no cut in his hand but my little saliva transferred to his hand. He has the habit of sucking his hand, so is there any chance of i infecting him through my saliva. I am asking because he is a 6 month old and his immune system and jaws and teeth are in development phase.
6. Would you be advising me against unprotected sex with my wife until i test and move ahead.
I know doctor all these question are anxiety filled ones-sorry about that

1. Thanks for the location information. With only 3% of infections there due to HIV2, it remains a near statistical impossibility your partner has it and was in the window period (under one chance in many million). And even if she had it, HIV simply is not transmissible by the kinds of contact you describe.
2. As I said, anxiety is a common reason to test. But no, you do not need PCR testing for a conclusive result, it's just a question of timing. PCR picks up ~95% of infections by 2 weeks.
3. Even with vaginal fluid inside the penis after 10-15 minutes of unprotected vaginal sex, if the female has HIV, the average transmission risk is one chance in 2,500. What can it possibly be with the trivial kind of exposure you describe? And we also have the information that she tested negative!!! What more do you need, for goodness' sake?
4. Just 3 doses of PEP doesn't make any difference.
5. Yes, this is an irrational, anxiety driven question. Since you cannot possibly have HIV, of course your child is not at risk. Even if you were infected, this sort of contact never transmits the virus. No children in the household of infected persons ever catch it unless there is sexual abuse. Never.
6. You should continue sex with your wife without worry.
Nobody on this forum ever needs to apologize for anxiety driven questions. But we DO expect users to carefully read, understand and believe our replies. It is not appropriate to remain anxious after reasoned, science based explanation that no risk exists.

Thank You Doctor.
May be I have been worried about this exposure for so long it feels like my mind is convinced that I am infected. I am questioning the authencity of the test- PCR and the Ab/Ag. I know only 2 follow ups are allowed and I am gonna use that hoping that could help me out of this anxiety and move ahead with my life.
1. Is there any chance the PCR test could be false negative. Can a human error or machine error gives out false negative result.
2. Just confirm to me once more doctor- My exposure was a no risk event- body to body rub, genital rub and masturbation with vaginal fluid. Even if the lady was infected with HIV, these exposure would not result in HIV transmission. And also please note I am uncircumcised.
3. In your experience have you came across transmission from this similar exposure?
4. I have this big anxiety if I made the right call stopping the PEP. In your first reply you also said you were glad I stopped the medication. I really hope I made the right call.
Doctor, this anxiety is really impacting me and I plan to see a therapist to address this concern. Your answers were really helpful and I can’t appreciate enough of your service.

1. No. Given the sophisticated technology of the test procedures, human error is not an issue.

2. Correct. Your circumcision status makes no difference.
3. No, never. And in the nearly 20 years of this forum and our preceding one, with thousands of questions from persons worried about an exposure resulting in HIV, not one has yet reported they were infected. You won't be the first. If and when it finally happens, it will be a real exposure with risk, not something like your story.
4. It was the right call.
I'm glad you're seeing a therapist: usually when someone is so anxious despite such reassuring advice, there is an underlying psychological problem, such as OCD, disordered perspectives on sex, or other mental health issues. I hope you follow through; you won't regret it.
