[Question #9814] Follow up concerns

Good afternoon doctors,
I have posted a few time on this website and I know some of the questions may seem repetitive but I do find reassurance hearing from you whichever doctor it may be when I have new or continuing concerns. I’m not sure if you’re able to few my previous posts but I had an exposure unprotected with a sex worker about 23 weeks ago. I have done several full panel tests all negative most recent 17.5 weeks from exposure. I have seen a urologist who believes it could be chronic prostatitis. I am still have urinary symptoms (dribbling after urination, bubbly urination, urine remaining in urethra after urination) as well as pain in the testicles that seem to come and go. I had genetic blood work done at my primary care (not std) and my creatinine levels were high which I assume is because I take creatine for the gym. The reason I am back on this site asking another question would be because I am anxiety ridden after looking that HIV can cause foamy urine as well as hepatitis c. I know that tests for HIV are conclusive at 6 weeks and I have had 3 negative Ag/Ab tests since then (9.5 weeks, 11.5, 17.5 weeks). Is there still no chance this is HIV? And if so is it possible this can be related to a Hep C infection? I know it can take 6 months for it to show on a blood test. I am just constantly reminded everyday of a mistake I made and it’s making it Extremely difficult to move forward with it when I still have these symptoms that seem to have started post exposure. I am sorry again for the repetitiveness but like I stated earlier I do find great relief on hearing your insight on the issues I am facing. Any insight as to what this may be is greatly appreciated. Welcome back to the Forum for the 4th time in the past 6 months regarding your encounter of concern. I'm sorry for your lingering concerns but the answers are not going to change- your tests are conclusive. Further, it appears that you have been searching the internet regarding your problems. This is a bad idea, particularly for an anxious person such as yourself-much of what is there will be misleading, either because it is out of date, taken out o context, of just plain wrong. The misinformation there is only going to fuel your anxiety. It may be worthwhile to discuss the chronic pelvic pain syndrome (CPPS) with your urologist- some of your symptoms are suggestive (the CPPS is well describe in Wikipedia)
Your symptoms (dribbling after urination, bubbly urination, urine remaining in urethra after urination) are not symptoms of an STI. Foamy urine is a non-specific finding that does not necessarily suggest HIV and your test results have unequivocally proven that you do not have HIV. Hepatitis C is not transmitted through sex with members of the opposite sex.
You need to move on. Clearly however your guilt and resulting anxiety over the encounter that occurred nearly 6 months ago is dominating your thoughts and, I suspect, hampering your normal, day-to-day life. Your test results are conclusive and there is no medical or scientific reason for continued testing. Please consider discussing methods to address your anxiety with a trained counselor or mental health professional- I say this out of concern and compassion for you, nothing more. EWH

Good morning Dr. Hook,
As I’ve stated before I do truly appreciate all that you guys do on here it is an outlet for people to express their concerns. I understand that I’ve posted 4 times in the last 6 months and you are correct in saying this incident has caused me a great deal of anxiety, but I do keep coming to this site because like others your insight and reassurance, repetitive or not, helps people get through these difficult times including myself. With that said, I am reminded of the incident every day as I still have CPPS symptoms almost 6 months later. The fact that these symptoms all happened directly after the incident keeps me thinking that somehow they must be related to the incident. I know that I’m doing myself no good by researching on the internet but I’m only doing so because I’m trying to figure out what exactly is going on so I apologize if all my posts and questions you have bothered you in some way. Im thankful for all the negative tests I received and this will likely be my last post to you. I don’t plan on any testing as you recommended and I am following up with my primary care doctor about my creatinine levels and the bubbly urine that I’ve had for over a month now. I have been beyond ready to move past the fact that this can’t be STD related but that’s why I come here to ask questions because I want to make sure the symptoms I am experiencing aren’t relative to any STI’s. I’ve explained the different medications I’ve taken over the 6 months including doxycycline alfuzosin and treatments for chlamydia and ghonnorrea and just want to clarify that those would have no effect on HIV test results correct?

Thanks for your thoughtful follow-up. Largely through a process of exclusion, I agree with you that this is likely CPPS and can verify that CPPS can follow a sexual encounter with a new partner. It remains mysterious but extensive research has demonstrated that CPPS, like other chronic prostate disorders are not STIs as you have verified through your testing. The mind has powerful impact on our experience of sensations and focus related to guilt/anxiety over a sexual encounter that one has come to regret. Sometimes discussions with a trained counselor as well as patience can be helpful.
I can assure you that not of the medicines you mention, nor anything else that is not specifically an anti-HIV medication would impact your test results. I urge you to believe the results... This is NOT HIV
