Hi Doctors,

I am a 36M who went to Columbia 8 months ago and had unprotected sex with an escort.  7 days post exposure, I took an std test and it came back with a HSV-1 IGG level of 2.69.  Since that time, I have taken multiple tests and my igg scores always land between 2.55 - 3.39 ( 4 tests). I have never had a coldsore or sores on my genitalia.  The escort sent me her std results 21 days post exposure and it was all non-reactive including hsv.  I trust her test, because she did not need to send me a test and the test came from a lab I called to verify and it had her real name, birthdate and home address, which I cross-referenced from my contacts there.

Would getting a hsv-1 igg test 7 days post exposure be too soon to detect hsv1?  Both my pcp and Urologist have told me that my HSV-1 was from way before, but reading forums a 2.69 IGG level 7 days post exposure would be new exposure?

What are the transmission risk of someone getting Ohsv1 From if they gave me oral and I have Ghsv1? Also, if I have Ghsv1, what are the risk of them getting Ghsv1 from me through vaginal sex?

If someone already has Ohsv1, what are their risks of contracting Ghsv1 from me if I have Ohsv1 and performed Cunnilingus?

The IgG test that was positive 7 days post exposure is unlikely related to the experience for a few reasons.  First, that's very quick to make that high an index value.  Second, HSV 1 is rarely transmitted via intercourse because if someone has HSV 1 genitally, it is rarely shed. 
It is statistically more like that IF this is a true positive, then it is likely oral.  Your index value is lower than some but would likely confirm with western blot, though not always. 
If they gave you oral sex and you already have oral HSV 1, then transmission to the genital area is highly unlikely. 
If you have oral HSV 1 and give oral sex to someone who also has HSV 1 orally, it is very unlikely that they would then contract genital HSV 1 from you.
is that what you are asking?  I'm a little unclear but I hope that is what you are asking. 
The problem with the study is that the direct skin to skin contact is the issue with transmission from someone who has a lesion, not so much saliva.  If someone has no lesion, perhaps something like this would be useful, but I sure wouldn't count on it

Hi Terri,

To clarify my encounter, it was unprotected vagina sex, kissing, she performed a rimjob on me(no blowjob) and I performed cunninglingus.  My fear is that I caught both Oral and Genital HSV-1 at the same time.  7 days afterwards through anxiety and guilt (I am married).  I went to take a panel test through Labcorp and it came back with HSV-1 at 2.69.  I have not cheated on my wife for 4 years, so I am trying to figure if my wife gave me HSV1 or I caught it from this encounter.  Sex worker sent me her panel 3 weeks after our encounter showing all negative, again I feel like I can trust her results, since she is high end and for her to send me her results with her real information included makes it believable. My issue is my wife has never shown signs of herpes nor have I.  My PCP, Urologist and Dermatologist all agree that it was not from my sex worker encounter.  Prior to my wife 4 years ago, I had sex with a lot of women unprotected.

With the information above, do you still agree that my sex worker encounter was not the cause of my HSV-1?

All 3 of my doctors told me to get off Valacyclovir daily, because they told me spreading oHSV-1 to my wife without an OB is unlikely, do you agree, can you elaborate? I have been off of valacyclovir for almost 2 months now.

From google pictures of genital herpes ob, do they always appear as vesicles, if not can you elaborate?

After getting off Valacyclovir, I have been getting headaches and neck stiffness for two months now, is this a sign of prodrome? 

From your experience if someone has gHSV-1, what is the likelyhood of having 1 OB and never having one again, I read that ghsv-1 normally has a true initial/primary OB and most people never have an OB again?

I would agree, but do we know that your wife is HSV 1 negative?  Has she ever had a cold sore in her whole life?  Have you?

Most often, HSV 1 would present as HSV 2 does - with water blister type lesions in the genital area.

If someone has not had a recurrence in the first year after having contract HSV 1 genitally, there is an 88% chance they won't have another.

So I seriously doubt that you she gave your both oral and genital HSV 1, when her test is negative and you have no symptoms.

I would agree, but do we know that your wife is HSV 1 negative?  Has she ever had a cold sore in her whole life?  Have you?

-I do not know if my wife is hsv-1 negative.  I do know that I was tested through planned parenthood 5 years ago and tested negative for everything.  However, one year span before, I met my wife 4 years ago, I had unprotected sex with atleast 15+ women non sex workers, but wouldn't a 2.69 IGG value be too low for something that happened 4-5 years ago?

I have taken the test at 7 days, 21 days, 12 weeks and 6 months post exposure with values between 2.55 - 3.39(besides the 1st test, I have been on Valacyclovir), would you recommend taking the Western Blot, or since I've taken the IGG test 4x already, I am a true positive?

I had a really bad outbreak of shingles covered almost the entire left half of my back; lasted 6 weeks during that 1 year span of sleeping around, which I took acyclovir for, would this create a false positive? 

From your experience, if a woman has recently caught gHSV1 and has an OB, I assume it would be major, noticeable and it would be unbearable; not heal within a week more so 2+ weeks, please elaborate?

From your experience are people naturally asymptomatic?  Since my test 8 months ago, I have been manic, I eat everything that people on reddit say not to eat for OBs, I am a firefighter in a big metro city, so there are a lot of emergencies, so I am always under high stress.  However, I still haven't had a OB.

The shingles is not going to make your IgG for HSV 1 look positive.  If a woman contracts HSV 1, she may or may not have symptoms.
I would suggest that you obtain the western blot to be certain you have HSV 1 at all.  But if we do that and you do have HSV 1, there is simply no way to know when you contracted this but I continue to believe that it was not from this encounter, due to the timing of the encounter and a positive test so soon afterwards and the fact that she showed you a negative test results.  Yes, some people are truly asymptomatic - the number for HSV 1 is about 70%
It is very possible that you've had this since childhood or it is a false positive or maybe you got it from a previous partner or even your wife.  
I'm sorry this is still so challenging for you.
