[Question #9955] HIV and STI Risk with Genital contact with Precum

Dear Dr, I am bisexual and mostly I been to have sex with man. Most of my exposure will be deep kissing , rubbing penises together with contact of Precum . There was once I was receptive anal and tested in 2021 negative . Since then , I never into anal. Does the mentioned activities on deep kissing , rubbing penises together with Precum is safe and no risk for HIV and other STI ? 
Will there be any symptoms if I m infected with HPV ? So far I tested all negative for STI in December 2022 i.e. 1 week after last exposure . I didn’t develop any STI or hiv symptoms. I also aware asymptomatic also will have in acute stage of hiv . I didn’t test for HIV since last test in April 2021 despite my exposure last year with several mans . I m paranoid and worried as many experts says no risk to test for HIV. I need assurance from the experts here . Many said need penetration for transmission . Otherwise no risk . I m really confused and help me to save my life .

Congratulations on your practice of safe, no risk activities.  While pre-ejaculate (pre-cum) does contain HIV, even if any of your past partners had untreated HIV infection, without penetration there is no known risk for infection.  Deep kissing, sexual rubbing (frottage) and non=penetrative contact with pre-ejaculate is a no risk event.  In fact, even mutual masturbation in which partners get each others' ejaculate on each other is a no risk activity for acquisition of HIV.  

HPV, which virtually all sexually active persons have, is not known to significantly increase the risk for HIV acquisition.  

If your only activities are those that you describe, there is no medical of scientific reason for testing.

Thanks for your reply Dr . It’s really put me in peace and out of anxiety . Because I always paranoid as hiv infected also no ARS . On HPV , will men develop any symptoms and how late they will develop ? 
If 5 months of post exposure , if I don’t have any oral or genital infection , can I conclude I do not have any STI ? 
Recent exposure after one month , I just had diarrhea one day , swollen lymph node in armpit bilateral . After 2 months I had just flu and sore throat . At 3rd months , I had again unilateral swollen lymph node in arm pit for 4 days n went off . All was not together and no generalized lymphadenopathy, so I assumed it’s not ARS or any STI . Am I right to say that ?

Nearly all unvaccinated sexually active persons will have HPV. It is typically an innocuous (not harmful) infection which goes away without treatment a year or two often it is acquired.  If you were going to develop visible genital warts (not all HPV infections go on to form warts, many are not visible), they would appear within several months of an exposure.   I encourage you not to worry about HPV.  If you are concerned, you should get the HPV vaccine which is highly effective for prevention of the most common HPV types.

Your symptoms are non-specific and certainly do not suggest the ARS.  When persons get the ARS, it most often occurs within 1-3 weeks after an exposure and is characterized by high fever, severe sore throat and muscle or joint aches, all of which occur at about the same time.

As I already told you, the exposures you describe are not risky for acquistion of HIV or STIs.  EWH

Thank you for the clarification Dr . You are making peace of mind for people as many online mislead . On the HPV , what is your recommended vaccine and no of dose and will that have side effects. I don’t want to infect my soul mate . 
For those STI test I did one week after last exposure is it conclusive as I never develop any symptoms on STI since my last exposure . 
Tested negative for 
HBsAg
HBsAb
Syphilis RPR 
HCV antibody 
Free T4 12.4 pmol/L 
Free T3 4.8 pmol/L
TSH 1.2 mIU/L
Do I need another STI test . As HIV is clearly noted no risk for acquiring . Thanks

We provide up to three responses to each client.  This is my 3rd response.  The thread will be closed after this response.

The HPV vaccine is widely available and prevents 9 of the most common and important types of HPV.  Here in the United States it is made by Merck and is widely available through doctors.  It is safe and effective, preventing well over 90% of the HPV infections covered.

Your syphilis (RPR) and Hepatitis B (HBsAg and HBsAB) tests are too early to rule out these infections but your risk for them is tiny.  These tests are not conclusive until about 6 weeks after an expsoure.  Your risk for these infections is miniscule and I would not seek them unless you develop symptoms.

Thanks Dr . Sorry what are typical symptoms of Hepatitis B and syphilis ? 
Would you recommend me for retest . I never develop any symptoms on genital , anus and mouth . Never had any discharge .

I told you your risk is low.  I see no need to retest.

Syphilis symptoms are open sires at a site of contact.  Hepatitis is often asymptomatic.
