[Question #9960] HIV and BIOTIN interference is wildly ridiculous and it's time to clear the air!

Welcome back to the Forum. We've discussed this before. Iagree with you- the biotin stuff is severely overblown.
Biotin-streptavidin indicator systems are used in many blood/serum/plasma diagnostic tests. This is certainly true for tests using strips for diagnosis but it does also pertain to liquid format lab tests as well on some occasions. Biotin is a normal part of the diet and many people take supplementary biotin but the idea that even supplementary biotin can lead to falsely negative blood tests is overblown. To put this in context, I do not quiz my patients on whether or not they are taking supplements when I do blood tests and I have no concern that this is a meaningful problem for the vast majority of persons. Too much has been made of the potential for biotin to impact test results. While in science one can never say never because strange things do happen, I would not worry whatsoever about biotin effecting HIV test results.
---I should add that, for those who thing they are "the one among millions"
who actually do have HIV and have tests which have been impacted by biotin they've taken, I would add that the half-life of biotin in the blood is about 15 hours, thus if 24 hours has passed since the last dose of biotin, blood levels will be too low to negatively impact the validity of tests.

Great reply, Dr. Hook. I just did my studying on it and believe that I have been misunderstanding the entirety of it until just recently. It is my belief, and personal understanding that you would need to ingest a MASSIVE dose of biotin within 1-3 hours before a blood draw when the levels are peaking to *potentially* interfere with an HIV 1-2 Ab/p24 Ag test. Something around 300mg might achieve that, and maybe not even then. I will ask this and then you may close the forum and I am going to board a plane to Aruba for vacation. In your educated and professional opinion, is biotin in the form of a B-Complex or multivitamin supplement, in the daily dose range of 300-400mcg (0.3-0.4mg), significant enough to alter or change the true results of an HIV 1-2 Ab/p24 Ag test laboratory blood test?
All the best and stay well,
C.W

I’m pleased you found my response helpful. I would not be concerned that the biotin contained in multivitamins ot B- complex supplements would meaningfully impact the reliability of tests for combination HIV antigen/antibody tests.
Enjoy your vacation. EWH
